We are seeking mission focused professionals to serve as a full time Case Coordinator responsible for providing in-home support services for foster parents and children.

As a Case Coordinator you would:

  • Create documentation for all services you provide in our Electronic Record System. Documentation includes Assessments, Diagnosis, Treatment Plans and Progress Notes, and Monthly reports. Overall documentation time is about 30%.
  • Provide in-home, direct services to families and children in your caseload. Generally, caseloads consist of 8-12 clients with whom you visit and support 2-3 times a month.
  • Participate in paid, rotating on-call schedule. Typically, 2 weeks on 6 weeks off, depending on staffing levels.
  • Develop and maintain consultative and liaison relationships with DSS and other members of the child’s treatment team (biological parents, school, etc.)
  • Travel within the Low Country and surrounding counties. Overall hours being worked outside of the office is estimated to be around 50%, which includes traveling to and from visits and time spend at the home conducting a home visit.
  • Work schedule will flex based on the needs of the families. This includes potentially working until 8pm several nights a month.
Position Requirements

What you bring to this role:

  • Master’s Degree in Social Work, counseling, psychology, sociology, or child & family studies is required. New graduates welcome!
  • 2 or more years of experience working with families and children, child welfare, and in-home therapies, required. Experience problem solving behavioral issues with parents, preferred.
  • Passion for serving youth, strong time management skills and working as part of a team.
