Transition Management Services - QP  

Category Assertive Community Treatment Team (ACT)  

Monday - Friday; 9:00am - 5:00pm

Essential Job Functions:

  1. Engages individuals in Tenancy Support Services as required by the service definition.

  1. Coordinates services and assures person-centeredness in all aspects of the service planning process.

  1. Develops holistic, integrated interventions.

  1. Participates in the initial development, implementation and on-going revisions of the Individuals’ Person Centered Plan (PCP), Crisis Plan, and Housing Support Crisis Plan

  1. Participate in “first responder” coverage and crisis response with the LME-MCO, provide crisis support to the individual by the initiation of WRAP or other crisis support plan. When required make immediate telephone contact with the individual and a face-to-face within 90 minutes after a crisis has occurred.

  1. Participate in the provision of structured rehabilitative interventions to individuals served such as, Assessment, Individual Housing & Tenancy Sustaining Services, Money Management and Entitlements, Activities of Daily Living, Personal Health, Wellness and Recovery, Promote Community Integration

  1. Coordinates communication with guardians, care coordination, housing stakeholders, landlords, Behavioral Health Services and other agencies serving the individuals being supported.

  1. Develop relationships with agencies that provide housing services, i.e., Housing Authority, permanent supportive housing providers, rapid re-housing providers, HUD-VASH, NC Housing Finance Agency, and relationships with local landlords to increase access to appropriate, safe, and affordable housing.

  1. Maintains case records as required by licensing and funding entities standards and regulations.

  1. Coordinates communication with guardians, care coordinators, behavioral health providers and other agencies serving the individuals being supported.

  1. Maintains good work habits regarding attendance, punctuality, appearance and effective working relationships with co-workers, LME-MCO staff, employers and individuals being supported.

  1. Accepts other duties as assigned by supervisor.
Position Requirements
  1. Bachelor’s Degree from an approved, accredited institution.
  2. Able to be credentialed as a Mental Health Qualified Professional or Associate Professional according to 10A NCAC 27G .0104.
  3. Knowledge of community housing resources.
  4. Have the knowledge, skills, and abilities to successfully provide TST to adults with mental illness and substance use disorders
  5. Minimum of two years’ experience working with the Adult Mental Health population
  6. Possesses current, unrestricted, state appropriate driver’s license.
  7. Possesses current, unrestricted vehicle insurance and provide own transportation for use in fulfilling job requirements.
