Description

ESSENTIAL FUNCTIONS:

  1. Schedule and coordinate prescriber appointments.
  2. Administer and monitor medications.
  3. Coordinate medical care and follow-up.
  4. Conduct home visits, sick visits and first-aid.
  5. Maintain consumer medical records.
  6. Provide medication education/nutritional counseling for consumers, families and staff.
  7. Consult with other program staff regarding consumer health issues and medication.
  8. Serve as liaison and advocate to community service providers for on-going, emergency and other forms of medical, mental health, crisis care, etc. for consumers

OTHER DUTIES: Run groups at partial care.

  1. Psych clinic with APN & RN on Wednesdays (All day)

      • Prepare for psych clinic, consult and provide information to program staff regarding consumer health and/or medication issues as well as the results of psych clinic when consumer is seen.
      • Input consumer's community medications into the Anasazi system and keep up to date (Joint responsibility with PC RN).
      • Communicate/follow up with Pharmacy as needed (Joint responsibility with PC RN).
    1. Review and sign weekly DSR's.
    2. Contribute to consumer IRP’s.
    3. Provide consumer medication training when needed.
    4. Document justification for the continuation of services and a recommendation for the appropriate level of care (Level A+, A, B or Supportive Housing).
    5. Administration of injections routinely ordered for consumers.
    6. Preparation of and contribution to nursing assessments.
    7. Read/review/sign and follow-up on MSRs (medical service reports).
    8. Conduct home and sick visits daily – weekly.
    9. Provide medication and/or healthcare training to residential staff as needed.
    10. Responsible for updating medical EHR/Anasazi.
    11. Attend IRP South, GH staff meetings and PC meetings as requested.
    12. Review and offer input on referral packets, potential admissions to the IRP South program. Attend interviews; participate in assessments for prospective consumers. Assist with consumers during their BV’s for possible admission. Give feedback to the Program Director.
    13. Work closely with residential staff to assist with the retrieval of medical records from ER/hospital visits. For consumers who have been hospitalized, coordinate information between home and facility. Coordinate discharges from facilities with facility staff and GH management.
    14. Assist GH staff when consumers must go to Crisis/hospital. Act as liaison with APN.
    15. In collaboration with the Treatment Team, help to develop the PRNA and CRNA with consumers in the Forensically Involved Supportive Housing Program on the 14th day of admission, every six months for the first year and annually thereafter.
    16. In collaboration with Treatment Team members, help to develop the PIRP which will allow for the provision of medically necessary services for up to 60 days after admission. The LPN will be involved in the development of the IRP no later than 60 days after admission, reviewed every 3 months and submitted to the IMR every 180 days for authorization.

Position Requirements

REQUIREMENTS: A NJ Nursing License, LPN License, and a New Jersey Driver's license. HS/GED minimum educational requirement plus at least 1 year experience in mental health. Ability to work flexible hours & to be on-call for emergencies & consultation

MANDATED PROGRAM REQUIREMENTS:

Behavioral Health Services: Qualification: one or more years of experience in the delivery of community based psychiatric services to consumers with serious mental illness. Valid NJ driver’s license and the ability to travel independently to a consumer’s residence, work site and/or hospital setting. The ability to be on-call and provide emergency response as needed and/or directed. BSN preferred. Bi-lingual capacity desired.
