REACH Team Leader

M-F (9-5) with rotating On-Call Duty nights & weekends

Position Summary: The position is responsible for primary administrative and clinical supervision of REACH Mobile Response Team. Position is responsible for shared rotation of a 24/7/365 crisis line which may include on-site response.

Essential Job Functions:

  • Implements REACH supports for individuals with intellectual/developmental disabilities and co-occurring mental illness or behavioral health needs.
  • Reviews staff service documentation for clinical appropriateness and accuracy.
  • Coordinates claims submittal and revenue collection from primary payor sources (e.g. third party payments).
  • Conducts retrospective service review to ensure services provided were in accordance with local, state and federal requirements.
  • Provides clinical and administrative supervision of REACH coordinators; consults with human resources as needed for employee performance and/or behavioral issues.
  • Oversees REACH Mobile support counselors in the provision of mobile crisis supports and assigns cases when referred.
  • Maintains linkages and relationships with community partners.
  • Provides 24/7/365 support to REACH coordinators according to on-call schedule, which may include on site response if needed.
  • Participates in required meetings with Team Members, REACH Director, CTH staff and other applicable parties.
  • Coordinates individual’s care with identified Case Manager or other Clinical Home Provider and/or appropriate CSB representatives and/or applicable crisis responder (Emergency Services, Mobile Crisis Teams, Law Enforcement, Hospital Emergency Departments, Walk-in Crisis, etc.).
  • Identifies and coordinates training and technical assistance needs for providers, natural supports and other components of the system of care.
  • Coordinates admissions/discharges and requests for appropriate individuals to/from REACH Crisis Therapeutic Home in collaboration with the REACH CTH Director and REACH Director.
  • Collects and enters required data and produces documentation on administrative and service delivery and overall utilization of REACH services.

Position Requirements

Minimum Qualifications:

  • Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP) – Minimum of a bachelor’s degree in human services or related field and no less than one year of fulltime relevant experience.
  • Graduation from an accredited college/university with a Master’s Degree in Counseling, Social Work, Psychology, or closely related field preferred.
  • Current, unrestricted Virginia license as an LPC, LCSW, or LMFT or closely related preferred.
  • Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs.
  • Two or more years of supervisory experience in a mental health/developmental disabilities setting.
  • Knowledge of the MH and I/DD systems of care.
  • Complete within specified time period the trainings as required by agency and service model.
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
  • Must demonstrate capacity to effectively communicate verbally and in written form.
  • Must have a valid, unrestricted, state appropriate driver’s license.
