Nurse - PACT

Description

What are we looking for in a Nurse?

Nurses collaborate with their assigned treatment teams to provide integrated, evidence-based treatment to clients. Nurses work closely with clients to administer medications, to provide education about prescriptions and general health needs, and to develop a treatment plan that reflects the clients’ goals. When possible, nurses will also coordinate with clients’ families or social supports to help position clients for success. Our programs have adopted trauma-informed principles to client care, so the nursing process will similarly take a respectful, client-centered approach. Training, continuing medical education, and regular supervision are provided as part of this position. This position is part of our PACT team, which is a service delivery model with the primary goal of recovery through community treatment. Its target population is those with challenging and persistent mental health issues. The PACT model is characterized by a team approach, shared caseload, time unlimited services, flexible service delivery and 24/7 crisis availability. This nurse position will have a schedule of Monday-Thursday 8am-7pm with participation in the on-call rotation schedule.

Nursing duties may include:
