Under the direction of Project Coordinator, researches, conducts and implements an Evidence Based Program Model to reduce pregnancy and birth rates for high-risk youth populations. Implementation of Teen Pregnancy Prevention involves various intervention activities, including: curriculum instruction, educational and informational presentations on sexually transmitted infections (STI), HIV/AIDS, increased contraceptive use among sexually active youth, recruiting and retaining youth, program monitoring, data collection and analysis, and reporting of performance measures.
Works with Project Coordinator in promoting, marketing, developing and participating in and maintaining stakeholder groups, to increase awareness in the community on reproductive health services and eligibility to receive those services.
Assists in the fulfillment of the agency's vision, 'For Those We Serve,' by empowering individuals to thrive as healthy, self-sufficient and contributing members of our communities.
DUTIES AND RESPONSIBILITIES/ESSENTIAL FUNCTIONS
Uses Evidence Based Program Model to reduce teen birth rates involving various activities, including; curriculum instruction, lesson plans, teachings, trainings, education and informational presentations.
Develops, delivers and performs health education and outreach activities in youth-targeted areas, including runaway, homeless, foster care, juvenile delinquent, and out of school.
Participates in, and maintains local stakeholder group.
Increases community awareness of Health Services and Education through publicity, presentations to local media, attendance at stakeholders meetings and conferences, events and media relations.
Provides progress and related reports.
Monitors and collects data for evaluation and performance measures.
Reviews outreach education materials and curriculum.
Administers surveys; local and state pre- and post-tests.
Assists with Clinic tours for youth providing information on services available through Family PACT, Medi-Cal and eligibility requirements.
Reviews literature, materials, and conducts research regarding teen pregnancy prevention to tailor program to participant needs and identifies strategies to increase access to healthcare and services.
Reviews and prepares outreach education materials and curriculum for use in teaching and presentations.
Participates in local community-based organizational events, including health and school fairs, festivals, church and related organized events.
Maintains log of activities and contacts. Collects and documents data information as required.
Provides information and referrals to youth in mainstream public schools, Juvenile Hall, Alternative School sites, Health Clinics and community-based organizations.
Organizes and maintains current health education library of brochures and pamphlets on diseases and health.
Identifies community resources and provides linkages.
Travels throughout Fresno County using personal vehicle, including transporting clients to various events.
Works occasional evenings and weekends.
Performs other duties as assigned.
