Description

Reporting to the Director, Client and Family Integrated Care, the Manager, Client and Family-Centered Care & Partnerships works within the program’s philosophy and framework to advance client and family-centred care (CFCC) and partnership at all levels of hospital decision-making. The Manager demonstrates leadership by modelling the hospital’s Mission, Vision & Values in everything they do. They are a lead consultant on the principles and best practices that foster staff, client and family partnership across governance and policy-making, research, education, program and service design, and clinical care delivery.

The Manager has accountability for Holland Bloorview’s award-winning Family Leadership Program and client and family-centred care initiatives that drive the hospital’s strategic plan, No Boundaries. Providing strategic guidance and consultation across the hospital community and broader health system, this role is central to facilitating the cross-program collaboration required to foster an authentic, quality-driven and equity-seeking organizational culture for client and family-centred care, and optimal client and family experience. Key partners to this role include clients and families; Child, Youth and Family Leaders (volunteers), students, clinicians, researchers, hospital administrators and a variety of community stakeholders.

KEY RESPONSIBILITIES

Program and Staff Management – Family Leadership Program

  • Fosters authentic engagement between clients, families and staff using leading principles and best practices
  • Provides strategic direction for the hospital’s volunteer-based Family Leadership Program, including oversight of the program budget, volunteer recruitment and retention strategy
  • Acts as a lead participant in the hiring, orientation, coaching and ongoing performance evaluation of program staff (e.g., development of individualized work plans for staff in accordance with Human Resources policies and procedures)
  • Responsible for leading a team of Family Partnership Specialists to recruit, support and partner with the hospital’s community of 150+ Child and Family Leaders (volunteers)
  • Leads program priority and goal-setting, project planning and implementation, and (re)design initiatives
  • Applies an equity, diversity and inclusion approach to identifying opportunities for, and the planning of, client and family partnership initiatives
  • Advances existing and new relationships with hospital programs (e.g., Teaching and Learning Institute, Bloorview Research Institute, Volunteer Resources, Communications, Foundation, Communications & Public Engagement)
  • Represents the Client and Family Integrated Care team at committees and task forces across the hospital
  • Manages the team that oversees the on-site lodging suites and guest experience for clients and families

Client and Family-Centered Care Education, Training & Evaluation

  • Partners with staff, educators, clients and families to co-design and deliver simulation-based client and family-centered care training, both within the hospital and across the health system
  • Consults hospital staff on client and family-centered care initiatives across the hospital and community
  • Develops and applies robust evaluation methods and indicators to measure the success of client and family-centered care and partnership across the hospital
  • Uses evaluation and data around client and family experience to make recommendations for improvement
  • Contributes to academic papers and knowledge production highlighting program initiative

Health System Outreach Partnership Building

  • Partners with stakeholders to develop and maintain new external partnerships to promote the Client and Family Integrated Care Program
  • Participates in relevant communities of practice, forums and strategic tables across the health-system
  • Delivers external presentations to promote the hospital’s model for client and family-centred care and client and family partnership
  • Consults external organizations on client and family-centered care principles and concepts, and leading models for client and family partnership
  • Advances the system-wide profile, scale and impact of the Family Leadership Program (e.g., increased partnerships, conferences/trainings etc.)

      Qualifications

      • Post-secondary degree or diploma in a relevant field (e.g., Human or Volunteer Resources, Community Development/Engagement, Social Work, Child & Youth Work), or the equivalent knowledge gained through lived experience with pediatric rehabilitation and/or disability
      • Masters degree in education, community or social services, or a related field (preferred, but not required)
      • Non-violence crisis intervention and/or de-escalation training (preferred, but not required)
      • Lived experience and/or training relating to and client and family-centered care and partnership practices (e.g., certificate programs, courses, conferences)
      • Lived experience with pediatric rehabilitation and disability, gained through first-hand experience at Holland Bloorview or beyond (preferred)
      • Proficient understanding and application of health equity, anti-oppression and/or population health frameworks and how these impact client and family experience
      • Strong project management skills and the ability to act independently
      • Thoughtful and energetic leader with a broad vision for the role of client and family partnership in hospital decision-making
      • Demonstrated skills in collecting and using data to assess programs and initiatives
      • Exceptional written and verbal (presentation) communication skills
      • Successful track record of accomplishment in leading change management initiatives, building new programs and teams in a complex setting (an asset)
