CLASS SUMMARY

Assists the staff psychiatrist in the prescription, administration and monitoring of client medications; performs routine physical and mental health nursing assessments, orders and dispenses prescribed medications and other medically based treatment services and provides supportive counseling. May monitor the medical needs of clients and/or administers methadone to clients. Prepares related documents and records. Incumbent may administer drug testing, T.B. testing and other medical screening.

DISTINGUISHING ELEMENTS OF THE CLASS:

I. IMPACT: The immediate health and safety of clients in the program are directly dependent on the work of this position. The incumbent performs routine physical and mental health nursing assessments. Failure by the incumbent to recognize key symptoms, make accurate assessments, oversee appropriate medications or referral could result in client death or dangerous behaviors. The Nurse is also called upon to respond to other medically based emergencies occurring at the facility. The incumbent is also responsible for determining fictitious medical/substance abuse problems presented by clients for the purpose of obtaining food and shelter or the evidence of incarceration.

II. LATITUDE: Routine work is generally performed independently subject to the review of the attending physician and on-going supervision of the nursing supervisor. Work is generally guided by agency and program procedures.

III. RESPONSIBILITY FOR THE WORK OF OTHERS: May supervise subordinate technicians on an assigned shift.

IV. INTERPERSONAL CONTACTS: Work involves continuous contact with clients and families/care givers; agency treatment and case management staff and pharmacists.

V. DIFFICULTY: Work requires the accurate assessment of clients experiencing a wide range of medical and emotional dysfunctions. Work entails strict adherence to technical medical administration techniques, policy and procedures. Accuracy and attention to detail are essential to the safe and efficient handling of the client medications and record keeping. Dealing with clients often involves patience and good judgment. Thorough and precise documentation is also required.

VI. COMMUNICATIONS: Work involves regular oral communication with agency staff, clients, physicians and related service providers. Routine letters are occasionally prepared.

VII. EQUIPMENT/FACILITIES RESPONSIBILITY: Incumbent is responsible for handling and dispensation of medication. The incumbent is responsible for the operation of a variety of medical equipment. Incumbent will use a pager and/or mobile phone equipment and a variety of routine office equipment.

VIII. WORKING CONDITIONS: Most work is performed in a clinic or residential setting and occasionally in a client's personal residence. The incumbent is frequently exposed to clients who often are physically unhealthy and emotionally unstable, hostile, aggressive and/or verbally abusive and present a risk of personal harm. Work involves the regular exposure to blood and other bodily fluids presenting a health risk to the employee. Work involves exposure to physically and emotionally unhealthy, unstable, hostile, aggressive and verbally abusive clients and who present a risk of personal harm. May be called in to work on different shifts and to perform housekeeping errands.

IX. KNOWLEDGE, SKILLS AND ABILITIES (KSA'S):

Thorough knowledge of the characteristics and precautions to be taken in the administration of methadone

Good knowledge of the medications commonly prescribed to the client population served and related psychiatric and medical terminology.

Good knowledge of the regulations governing the security, dispensing, disposal of and storage of controlled substances.

Good knowledge of supportive counseling practices.

Good knowledge of the standards for record keeping related to medication dispensing.

Good knowledge of the behavioral characteristics of individuals with substance abuse problems.

Ability to accurately maintain client medical records and prepare reports.

Ability to accurately maintain and monitor a methadone inventory.

Ability to establish a rapport with clients.

Good knowledge of the laws, guidelines, and legal procedures relating to mental health care or substance abuse treatment to include HIPAA and 42 CFR Part 2.

X. FORMAL TRAINING AND WORK EXPERIENCE EXEMPLIFYING KSA'S: Graduate of an accredited LPN school of nursing with three (3) + years of experience in the administration of medication or in a substance abuse treatment, psychiatric setting or long term care facility including the dispensing of narcotic medication.

XI. LICENSURE: Current and valid State Licensure as a Licensed Practical Nurse from the Virginia State Board of Nursing.

XII. PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Processes medications order for clients as prescribed by a clinic psychiatrist.

Secures, stores, and disposes of medications and prepares related records.

Administers and/or delivers and records medications to clients.

Provides routine supportive counseling to clients in the course of medical assessment and treatment.

Makes routine assessments of client behavioral and medical status and prepares relating documentation.

Responds to calls for routine medical information.

Maintains stock medicine inventories and reorders as required.

Conducts client medical intake and follow up interviews and performs physical assessments.

Schedules and assists the physician with medical examinations.

Performs skilled nursing duties as ordered by physicians

Dispenses methadone, maintains and secures methadone inventories, and prepares related client and administrative records.

Collects and delivers laboratory specimens for analysis.

Follows up on all medical concerns, including laboratory results.

Oversees the self-administration of client medications.

Does related work as required.

XIII. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. Some standing, walking, moving, climbing, carrying, bending, kneeling, crawling, reaching, and handling, pushing, and pulling.
