CLASS SUMMARY:

The Incumbent ensures continuity of directly provided and contractual services to agency’s individuals with intellectual and developmental disabilities. The position will assist support coordinators in meeting the Licensing and Medicaid regulations.

DISTINGUISHING ELEMENTS OF THE CLASS:

I. IMPACT: The Community Support Services Specialist will assist the Support Coordinator in performing administrative task in the day to day operation of managing individuals with varying needs. This position has the responsibility for assisting the Support Coordinator with transactional activities that are administrative in nature. The incumbent is system oriented and has the knowledge of the transformed Medicaid Waiver System.

II. LATITUDE: A Community Support Specialist performs work task independently with only occasional consultation with their supervisor. A Community Support Specialist is expected to provide administrative duties that are essential in support coordination services. Also, they must address administrative and or policy issues without individual consultation with their supervisor. Work is guided by agency policies, state/federal regulations, and complexity of cases. The incumbent is expected to make considerable independent decisions.

III. RESPONSIBILITY FOR THE WORK OF OTHERS: None.

IV. INTERPERSONAL CONTACTS: Work involves regular contact with providers internal and external to the agency, individuals, families, care givers, private providers, contractual program staff, local human services and health agencies, physicians, attorneys, court personnel, police, jail personnel, hospital staff, residential treatment and vocational rehabilitation personnel, state and federal service agencies, school personnel, employers, landlords, community groups and organizations and legislators.

V. DIFFICULTY: Work involves assisting the support coordinator in meeting the daily demands of their caseload in the areas of medical, behavioral, employment, and housing. The community support specialist is expected to provide activities that have little or no impact on the quality of life of the individuals. The community support specialist will not have a caseload but will be expected to assist occasionally with the most difficult clients and remain flexible in meeting the demands of administrative needs of the support coordinator.

VI. COMMUNICATIONS: Constant oral communication by phone and in person with clients and other parties involved. Should have an acceptable level of efficiency with Microsoft Office Suite include Word, Excel and PowerPoint. Frequent standardized reports, correspondence and client service plans and summaries are written by incumbent. The incumbent may occasionally address community groups, peer groups and directors, managers, and coordinators in the agency.

VII. EQUIPMENT/FACILITIES RESPONSIBILITY: Incumbent frequently operates agency or personal vehicles to visit individuals, families, private providers and behavioral health agencies. Incumbent may also use mobile telephone equipment.

VIII. WORKING CONDITIONS: Work frequently involves traveling to various sites to include conditions located in neighborhoods of varying security. Work may also take incumbent to homeless shelters, jails, and out-of-town locations. Incumbent is expected to respond during work hours to calls for assistance from the support coordinator and or supervisor. This position will provide may provide transportation, home visits, medical & employment follow up, and contact with other providers. The incumbent is exposed to hostile and/or aggressive behaviors presenting a personal risk of bodily harm.

IV. KNOWLEDGES, SKILLS AND ABILITIES (KSA'S):

Ability to use an automobile. DMV driving record must be of sufficient quality to be accepted by agency automobile insurance underwriterGood skills in the use of a computer and other office machines.

Ability to establish effective working relationships with other employees.

Ability to communicate effectively, orally and in writing

Ability to communicate with individuals, families, caregivers, and professionals.

Good knowledge of community and agency resources and programs and of regional and state resources for individuals with a Developmentally Disability.

Good knowledge of human development and behavior.

Ability to establish and maintain rapport with individuals and their families.

Ability to establish and maintain effective working relationships with other employees and representatives of various public and private agencies.

Knowledge of Adult DD Medicaid Waiver and Department of Health Services.

Ability to view and address the total spectrum of service needs.

Ability to be a problem solver.

X. LICENSURE: Valid Virginia Driver's License.

XI. PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Assist in discharge planning, coordinating services and or completing referrals to meet medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational, and advocacy needs.

Advocates for the needs of individuals within and outside the system and serves as a liaison between the individual, provider, and care provider.

Assist individuals in obtaining basic resources such as shelter, food, medical services, and

disability benefits by completing applications.

Assist in preparing paperwork for completing Medicaid Applications, Mortality Review Packets, Respite and Emergency placements.

Completes referrals to appropriate service providers.

Prepares memos, correspondence, and reports related to support coordination and reports to supervisors per agency policy and state regulations.

Enters documentation as required in the Electronic Health Record.

Reports health and safety issues to DBHDS and the Community Resource Consultant.

Assist individuals in touring employment and housing options within the community.

Assist the Support Coordinator with securing Exclusionary Criteria/Citations for all providers.

Completes Regional Support Team (RST) Referrals for individuals and participate in the staffing of cases with DBHDS.

Assist in securing OT, PT, SLP, and Behavior Consultation for individuals.

Contacts MCO’s for individuals in need of referrals and or linkage for ancillary services.

Participates in completing housing vouchers with DBHDS.

Coordinates Capacity Evaluation referrals.

Assist in completion of the National Core Indicator Surveys.

Utilizes e-Calander to organize job related task

Provides transportation for individuals to appointments and assist in emergency situations.

XII. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. Some walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, and handling, sitting, standing, pushing, and pulling.

XIII. DRIVING REQUIREMENTS: Valid Driver’s License. Transports individuals as required.
