Position Transgender Health Navigator 2018

Description

Function:
The Transgender Health Navigator provides health navigation services, HIV, STI and Hepatitis C screenings, risk reduction activities, community outreach, community level interventions, and events targeting transgender individuals living with or at high risk for HIV/AIDS.

Duties:

  1. Provide direct services
  • Perform health navigation services for those in need, including making supported referrals to address the full spectrum of client needs and providing appointment accompaniment. Ensure linkage to primary care, medical case management, behavioral health treatment, PrEP, nPEP, disease intervention specialists and other needed services.
  • Conduct in-reach and outreach (e.g. bars, nightclubs, public sex environments, shelters, community health centers, Pride events, etc.) to find, engage and retain the target population in care and services.
  • Provide written and verbal harm reduction information on safer sex strategies, hormone and silicone use, substance use, disclosure and HIV, STI and Hepatitis transmission. Distribute sterile syringes and harm reduction materials according to agency and MDPH protocols.
  • Provide education on and navigation services to gender affirming procedures, hormone treatment and risk reduction strategies.
  • Coordinate referrals to internal and external services as well as other community resources.
  • Conduct HIV/HCV/STI on and offsite screening, perform phlebotomy, and risk reduction assessments and counseling sessions.
  • Provide extended or multi-session risk reduction sessions for HIV-positive and high-risk HIV-negative clients when appropriate, including through the use of evidence-based interventions such as ARTAS.
  • Facilitate group and community level interventions.
  • Provide community-based transgender trainings or workshops as appropriate.

  • Complete appropriate and accurate documentation in a timely manner.
  • Attend staff, department meetings and individual and clinical supervision as required.
  • Collaborate with internal and external colleagues performing related work.

  1. Participate in planning and implementing new interventions
  • Participate in program planning through interdisciplinary team meetings.
  • Attend continuing education seminars, staff in-services and stay abreast of relevant literature to maintain familiarity with trends in prevention services.
  1. Meet agency participatory expectations
  • As required, work a flexible schedule, including some evenings and weekends, as needed to provide services and perform outreach or coordinate events in ways that best reach at-risk communities.
  • Adhere to all agency and departmental policies and procedures
  • Participate in quality assessment and improvement activities as requested
  • Adhere to the highest principles of client confidentiality
  • Adhere to established safety policies, procedures and precautions; enforce agency's policies and clinical procedures, including safety and crisis procedures.
  • Attend all required meetings, in-services and professional trainings
  • Maintains professional competence necessary to perform job responsibilities; maintains and provides agency with records of continuing education activities

Position Requirements

Requirements:

  • Minimum Bachelor’s degree and/or two years of work experience with the target population
  • Strong understanding of and interest in transgender health issues
  • Phlebotomy certification/experience or a willingness to be trained in phlebotomy
  • Strong interpersonal communication skills
  • Strong ability to manage clinically and emotionally challenging situations with utmost professionalism, care and discretion
  • Ability to provide precise, well-written documentation
  • Experience providing HIV education, testing and counseling a plus
  • Demonstrated computer skills (Microsoft Outlook, Word and Electronic databases)
  • Ability to handle multiple tasks and prioritize assigned duties
  • Ability to work independently and in teams
  • Experience working in an ethnically, culturally, and racially diverse environment preferred
  • Ability to meet deadlines and work within a team-based environment
