Description

CLASS SUMMARY:

This class provides in-home and out-of-home education, behavioral interventions, training and crisis management services to children and adolescents with moderate to severe emotional disturbance, intellectual developmental disability or substance use disorder and their families. Family service work includes the provision of crisis intervention, behavioral interventions, participation in treatment planning, outreach, service provider linkage, monitoring, advocacy, daily living support, respite, independent living skills, vocational instruction, medication monitoring, supportive counseling and positive role modeling.

DISTINGUISHING ELEMENTS OF THE CLASS:

I. IMPACT: The incumbent’s work is to provide services to the client and family in order to maintain client within the home and/or community.

II. LATITUDE: Day-to day work is performed under the supervision of the Clinical Services Supervisor. Work is guided by an individual service plan, a FAPT service plan, program policies, guidelines and instructions from the Supervisor and treatment recommendations from service providers, i.e. in-home therapist, outpatient therapist, case manager, service coordinator, FAPT team.

III. RESPONSIBILITY FOR THE WORK OF OTHERS: None.

IV. INTERPERSONAL CONTACTS: Work involves direct and continuous contact with clients and other staff in planning and providing client services. Regular contact with family members, in-home therapists, case managers, outpatient providers and service coordinators. Staff will attend regularly scheduled FAPT review staffings.

V. DIFFICULTY: Appropriately relating to a variety of client and family personality types requires use of judgment and discretion. Application of learned skills for the targeted population of moderate to seriously emotionally disturbed youth, mentally retarded, or youth with substance abuse issues and their families are required. Non-violent crisis intervention techniques may be warranted and applied in some cases.

VI. COMMUNICATION: Incumbent will communicate effectively, in writing and orally, to counselors, clients and family members, and staff of other service agencies. Incumbent must also maintain client progress notes and daily logs related to billing and accountability for time worked.

VII. EQUIPMENT/FACILITIES RESPONSIBILITY: Incumbent operates personal car to visit client and contact community resources. Incumbent will also use pager, computer and various office equipment, including a copier, shredder, fax machine, and printer.

VIII. WORKING CONDITIONS: Work frequently involves traveling and visitation to client homes of varying sanitary and health (emotional and physical) conditions. Incumbent may be exposed to hostile and/or aggressive client behavior, presenting a personal risk of bodily harm. Work for documentation purposes is performed in a typical office environment.

IX. KNOWLEDGE, SKILLS AND ABILITIES (KSA’S):

Good knowledge of non-violent crisis intervention.

Good knowledge of family life.

Knowledge of parenting skills and behavior management techniques.

Knowledge of basic counseling techniques.

Knowledge of the Family Assessment and Planning Team process for Newport News and Hampton.

Knowledge of various agency systems, i.e. CSA, courts, Social Services, CHINS, etc.

Demonstrated skill in working with problem children and adolescents.

Very good oral and written communication skills.

Ability to establish and maintain effective relationships with clients, families and staff.

Ability to work as an active member of a treatment team.

Ability to meet documentation deadlines.

Ability to identify and access community resources.

X. PRINCIPLE DUTIES AND RESPONSIBILITIES OF CLASS:

Provides in and out-of-home support/education for clients under the supervision of the Clinical Services Supervisor, Mentor and Independent Living Program.

Monitors and records client behavior and documents client progress according to existing standards and policies and to assure protection of client confidentiality.

Implements Individual Service Plans.

Provides non-violent crisis intervention for clients and families.

Cooperates as an active treatment team member.

Actively participates in group and/or individual supervision.

Attends scheduled FAPT staffings and reports client’s progress and makes recommendations.

Provides training in or reinforcement of functional skills and appropriate behavior related to the individual’s health and safety.

Provides training in activities of daily living and use of community resources; assistance with medication management; and monitoring health, nutrition, and physical condition.

Communicates regularly with service providers.

Performs billing tasks (SALS).

Does related work as required.

Transports clients as necessary.

XI. LICENSURE: Valid Driver's License.

XII. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with clients, families, service providers and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively apply non-violent crisis intervention techniques to prevent physical harm to self or others. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a personal computer, calculator, copier, and fax machine. Some walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, and handling, sitting, standing, pushing, and pulling.
