CLASS SUMMARY

Incumbent ensures continuity of directly provided and contractual services to agency’s individuals with intellectual and developmental disabilities. Support Coordinators assist individuals with their basic physical, financial, social and personal needs. Support Coordination services for the individuals include development of person centered plans, service provider linkage, monitoring, advocacy, counseling crisis intervention, daily living support, and enhancing community integration.

DISTINGUISHING ELEMENTS OF THE CLASS:

I. IMPACT: Support Coordinators are the central professional with whom people with disabilities and their families develop relationships. The roles as case manager is to assist the individual and family to understand the services system to help make informed choices about services and supports, and to engage in high quality, person centered planning. The health and welfare of the intellectual and developmental disabilities population served are directly dependent on the work of this position. The incumbent assesses needs and develops outcomes based on assessments and refers individuals to meaningful services, follows DOJ requirements including ongoing monitoring success of the individual. This position has the responsibility for advocating, ensuring health and safety needs, reporting all critical incidents, and adhering to regulatory requirements.

II. LATITUDE: A Support Coordinator performs work task independently with only occasional consultation with their Supervisor. A Support Coordinator is expected to understand Department of Justice requirements and can make clinical decisions regarding their need for enhanced monitoring of services. Also, they must address administrative and/or policy issues without individual consultation with their Supervisor. Periodic review of case management records is made by the Supervisor. Work is guided by agency policies, state and federal regulations, and complexity of cases. The incumbent is expected to make considerable independent decision making.

III. RESPONSIBILITY FOR THE WORK OF OTHERS: None.

IV. INTERPERSONAL CONTACTS: Work involves regular contact with providers internal and external to the agency, clients, families, care givers, private providers, contractual program staff, local social services and health agencies, physicians, attorneys, court personnel, police, jail personnel, hospital staff, residential treatment and vocational rehabilitation personnel, state and federal service agencies, school personnel, employers, landlords, community groups and organizations and legislators.

V. DIFFICULTY: Work involves assessment of client needs and the development of the Person Centered Plan specific to the assessment outcomes. The Support Coordinator is expected to demonstrate progress or success with clients in the areas of Health and Well Being, Community Inclusion, Choice and Self-Determination, Living Arrangement Stability, Day Activity Stability. Work requires and involves the Support Coordinator knowing and monitoring the client’s health status, any medical conditions, and his medications and potential side effects, and assisting the individual in accessing primary care and other medical services. Work involves enhancing community integration, collateral contacts, thirty day face to face visits, and understanding capabilities of clients to meet needs and preferences without placing them at risk of serious harm. The incumbent works with hostile or aggressive clients and is expected to respond using approved techniques and agency resources. Lack of available resources requires use of creativity and innovation to deal with specific client needs, often in crisis situations. The case load of a Support Coordinator will generally contain the most difficult clients.

VI. COMMUNICATIONS: Constant oral communication by phone and in person with clients and other parties involved. Frequent standardized reports, correspondence and client service plans and summaries are written by incumbent. The incumbent may occasionally address community groups, peer groups and directors, managers, and coordinators in the agency.

VII. EQUIPMENT/FACILITIES RESPONSIBILITY: Incumbent frequently operates agency or personal vehicles to visit clients and contact community resources. Incumbent may also use mobile telephone equipment.

VIII. WORKING CONDITIONS: Work frequently involves traveling and visitation to individuals homes of varying sanitary and health (emotional and physical) conditions located in neighborhoods of varying security. Work may also take incumbent to homeless shelters, jails, and out-of-town locations. The incumbent is exposed to hostile and/or aggressive individuals with behaviors presenting a personal risk of bodily harm. Incumbent is expected to respond during off hours to calls for assistance for clients for whom incumbent has primary responsibility.

IX. KNOWLEDGES, SKILLS AND ABILITIES (KSA'S):

Thorough knowledge of IDD, Autism, and other Developmental Disabilities, physical, emotional, and sensory disorders.

Ability to determine Department of Justice eligibility criteria for individuals on a continuous basis.

Thorough knowledge of interviewing and supportive counseling principles and techniques.

Thorough knowledge of social casework principles and techniques.

Thorough knowledge of community resources, regional and state resources, and providers for intellectual developmental disabled.

Thorough knowledge of an individual’s health status, medical conditions, medications and potential side effects.

Knowledge of DBHDS, DMAS, Human Rights, and local support services.

Knowledge of Medicaid Waiver Regulations.

Ability to work independently.

Assures Coordination of services and service planning with all providers and human service agencies.

Ability to plan for transitions in the individual’s life.

Ability to collect and compile assessment information and Person Centered Plans.

Ability to prepare families for Independent Reviews, National Core Indicator Surveys, Family Support funding, and any pertinent DBHDS changes.

Ability to understand the differences in Provider Licenses.

Ability to establish and maintain rapport with individuals and their families.

Ability to assist the individual directly to locate, develop, or obtain needed services, resources, or public benefits.

Ability to maintain effective professional relationships with employees, public and private agencies.

Ability to make sound clinical decisions for individuals with complex needs and offer appropriate housing options.

Ability to assess an environment and identify potential risks, injuries, needs, or change in status.

Ability to communicate effectively and prepare related documentation.

Ability to prepare and maintain narrative and statistical records and to prepare related reports.

Ability to determine the need for a Capacity Evaluation of an individual.

Ability to provide guidance on differences of an Authorized Representative, Guardian, and/or Custodian.

X. LICENSURE: Valid Virginia Driver's License.

XI. PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Coordinates services to meet medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational, and advocacy needs.

Develops Person Centered Plans for individuals and revises them accordingly. Participates in individual’s quarterly reviews and monitors all supports to ensure quality services are being provided and consistent with all assessments.

Advocates for the needs of individuals within and outside the system and serves as a liaison between the individual, provider, and care provider.

Reports all risk, injury, need, or change of individual’s status to the appropriate licensing agency.

Assists clients in obtaining basic resources such as shelter, food, medical services, and disability benefits. Assist with client moving and arranging transportation.

Monitors individual’s progress and ensures the individual is attaining outcomes according to discharge protocols.

Prepares documentation specific to Department of Justice criteria.

Provides supportive counseling to individuals and families, provides emergency crisis resources, and refers to specialized programs.

Completes referrals to appropriate service providers and links the individual to the services.

Participates in discharge planning and develops transitional plans for individuals. Prepares memos, correspondence, and reports related to support service coordination/case management per agency policy and state regulations.

Enters all documentation in the Electronic Health Record. Attends and testifies at guardianship hearings and other legal proceedings as needed.

Reports all health and safety issues to DBHDS and the Community Resource Consultant.

Participates in DBHDS Independent Reviews, VOPA, Commitment Hearings, Regional Support Team, and Post Move Monitoring Reviews.

Transport clients as necessary.

XII. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. Some walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, and handling, sitting, standing, pushing, and pulling.
