We are seeking tenacious, confident, and compassionate professionals to serve as a full time Case Mangers responsible for being the front line in providing Community Based Prevention Services for families as an alternative to DSS involvement.

As a Case Manger you would:

  • Interpret and use, sometimes limited, information provided by DSS to determine a child’s safety, risk, and wellbeing.
  • Work with families who are struggling with very difficult challenges including domestic violence, substance abuse and mental health conditions.
  • Assume responsibility for the assessment of the families to determine if they require additional services while developing and maintaining ongoing, in-home, case management of your caseload.
  • Create documentation for all services you provide in DSS Portal and SAFY Electronic Record System. Develop and implement Treatment Plan based on assessments.
  • Develop and maintain consultative and liaison relationships with external providers (i.e. Probation Officers, Physicians, Guardian ad Litem, Therapists, etc.), and DSS and other members of the child’s treatment team.
  • Travel extensively throughout Pickens and part of Greenville County. Overall hours being worked outside of the office is estimated to be around 70%, which includes traveling to and from visits and time spent at the home conducting visits and completing assessments.
  • Set your schedule in collaboration with the team. Generally, we work standard hours of around 9am – 5:30pm, however there are times when you may need to work outside of these hours (including weekends), in order to meet the needs of the families.
Position Requirements

What you bring to this role:

  • Master’s degree in social work, counseling, psychology, sociology, or child & family studies is required.
  • New graduates are welcomed! Must be organized, ability to track and follow-up with clients/families. Ability to handle conflict effectively. Demonstrated engagement skills to get families to accept services. Possess great comfort going into the home independently. Time management skills to take care of administrative and clinical duties.
