REACH Therapeutic In-Home Counselor

This position is generally 9-5 M-F.

· The REACH Therapeutic In-Home Counselor provides crisis stabilization services to individuals with co-occurring intellectual/developmental disabilities and mental or behavioral health needs within the individual's home or other community setting. The REACH Therapeutic In-Home Counselor participates as part of comprehensive REACH services that collaborate with individual’s primary provider and clinical home, community crisis services, family, and others.

· Plans, structures and prompts social and leisure time activities, including activities that build skills for behavior management, stress management, communication, adaptive living and daily living.

· Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports.

· Participates in development and implementation of persons’ cross system crisis prevention plan.

· Completes documentation within required timeframes

· Participates in weekly multidisciplinary team meetings

· Provides support at the REACH Crisis Therapeutic Home when there are no individuals requiring support in the community.

· Other duties as assigned.

· Frequent travel is required throughout Health Planning Region 1 which covers the following locations relative to their CSB offices in central Virginia: Region Ten CSB (Charlottesville), Horizon BH (Lynchburg/Bedford), Harrisonburg-Rockingham CSB, Rockbridge CSB (Lexington), Valley CSB (Staunton/Waynesboro), and Alleghany-Highlands CSB (Clifton Forge/Covington). Reliable transportation is required.

· Mileage reimbursement is provided. Company-issued laptop and cell phone stipend are provided.

Position Requirements

Qualifications: Must possess at least a bachelor's degree in a human services field, including, but not limited to sociology, social work, special education, rehabilitation counseling, or psychology. Must meet Virginia DBHDS criteria as a Qualified Professional: intellectual disabilities (QIDP) and/or mental health (QMHP) – preferably both. At least one year of experience in the field of intellectual and developmental disabilities, including programming for individuals with significant behavioral support or mental health needs, is required.

Candidates with experience working in mobile crisis intervention services, working with individuals with challenging behaviors, positive behavior support (PBS), and/or applied behavior analysis (ABA) are encouraged to apply. Understanding of the system of care for individuals with developmental disabilities and mental health needs is necessary.

Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, web-based data entry, etc. Must demonstrate capacity to effectively communicate verbally and in written form. This position carries a high degree of autonomy and will require an individual that is highly self-motivated. Candidates must be flexible, patient, and comfortable with teamwork and diplomacy.

Must possess a current, unrestricted, state-appropriate driver’s license and current personal vehicle insurance.
