About the Organization The mission of Fenway Health is to enhance the wellbeing of the lesbian, gay, bisexual, and transgender (LGBT) community and all people in our neighborhoods and beyond through access to the highest quality health care, education, research, and advocacy. Fenway Health was founded in 1971 and has grown from a less than 10 person operation in the basement of a building near the Boston Fens to one of the largest LGBT-focused organizations in the world. It consists of three main centers:

• Fenway Community Health Center, our clinical arm, which is a federally qualified health center that operates three clinic sites (Fenway:1340, Fenway:South End, and the Sidney Borum, Jr. Health Center) in the Boston area and provides over 30,000 patients with primary and specialty medical care, behavioral health care, optometry care, and dental care;

• The Fenway Institute, our research/training/education/policy development arm, which houses numerous research initiatives as well as the National LGBT Education Project, the LGBT Aging Project, the National Center for Innovation in HIV Care, and the Center for Population Research in LGBT Health; and

• AIDS Action, our HIV/AIDS service organization, which provides nonclinical services (case management, housing and legal assistance, outreach, peer support) to people living with HIV/AIDS and operates numerous programs focusing on HIV/STD prevention/testing/treatment, including the Access (drug user health program), TransCEND (transgender peer support and risk reduction program), and Youth on Fire (drop-in center for homeless and street-involved youth) programs.
 
Position High-Acuity Substance Use Case Manager

Description

Function:
Under the general supervision of the Behavioral Health Specialist/Case Management Supervisor, the High-Acuity Substance Use Case Manager provides case management and health navigation services and coordinates referrals to community resources for patients enrolled in Fenway Health's Medical Assisted Treatment Program, Addiction Recovery and Wellness Program, and/or AAC's Access Drug User Health Program. The High-Acuity Substance Use Case Manager will act as a motivator, ally, collaborative problem-solver, advocate and health navigator to increase patient retention in appropriate office-based care, support patient adherence to prescribed medications and support patient engagement with treatment programs as appropriate.

Representative Duties:

  1. Provides case management services to patients and clients with high level of need related to substance use disorders:
  • Provides services to patients and clients who are identified as high risk due to substance use and behavioral health problems
  • Interacts with patients and clients in office and community settings (home visits, community locations, hospitals, other program sites)
  • Provides high level of support to ensure barriers to treatment and medication compliance are mitigated
  • Provides on-going support to patients and clients as appropriate and conducts outreach to help retain them in care or to encourage them to utilize additional necessary services
  • Uses Motivational Interviewing to assess readiness and preparedness to change
  • Provides supportive counseling to assist patients and clients with achievement of goals
  • Assists patients and clients with addressing and mitigating social determinates of health that are barriers to wellness
  • Forms productive, empathic working relationships with those in assigned caseload
  • Meets established service delivery targets
  1. Provides medical case management services:
  • Provides case management services to medical department patients, with an emphasis on a caseload of those with a high level of need
  • Reviews patients' and clients' medical, mental health and social history, identifies problems and appropriate referrals appropriate in a timely, courteous manner.
  • Links patients and clients when appropriate to mental health and substance abuse services, including step-up services in the community; referrals may include further assessment/evaluation by licensed clinician
  • Provides information to patients/clients about available community resources and assists clients to determine eligibility and obtain services
  • Assists patients/clients with applications for enrollment in entitlement programs, including assisting them with timely completion of routine paperwork.
  • Assists Behavioral Health Specialists with timely completion of applications and follow-up
  • Adheres to NASW Code of Ethics
  1. Collaborates with staff from other departments and agencies:
  • Develops and maintains resource listings appropriate
  • Participates in community councils, boards and groups as appropriate to further departmental objectives
  • Works with Medicaid, Medicare, Welfare, Social Security, housing, educational, day care and other institutions to assist clients
  • Consults with a variety of health center and community staff (Behavioral Health staff, Medical staff, MAT Program staff, Addiction Recovery and Wellness staff, Access Drug User Health staff, nurses, Nutritionists, Home Health Aides, etc.) about specific clients and general approaches to patient treatment
  • Maintains updated knowledge of recovery/substance use resources in the community
  1. Performs and completes administrative tasks in a timely and efficient manner:
  • Coordinates with behavioral health staff on comprehensive treatment planning
  • Writes case notes for each client interaction
  • Assists medical teams with case management administrative tasks as needed
  • Participates in staff and team meetings and conferences
  • Participates in professional activities to maintain professional standing
  • Participates in program reporting and accountability as required by funding agencies
  1. Meets agency participatory expectations:
  • Suggests and participates in continuing quality improvement efforts
  • Adheres to all agency and departmental policies and procedures
  • Participates in quality assessment and improvement activities as requested
  • Adheres to the highest principles of patient and client confidentiality
  • Adheres to established safety policies, procedures and precautions; identifies potential or actual unsafe situations in the environment and takes measures to rectify the situation
  • Attends all required meetings, in-services and professional trainings
  • Attends all regularly scheduled research meetings, trainings, forums and pertinent FCH staff meetings
  • Maintains professional competence necessary to perform job responsibilities; maintains and provides agency with records of continuing education activities
  • Serves on agency committees and in professional organizations when requested
  1. Performs related duties as required

Position Requirements

Requirements

  • Bachelor degree in social work or related field required, MSW preferred
  • CADC or LADC-II preferred
  • Ability and interest in working with patients utilizing a broad range of treatment approaches, including medication assisted treatment, 12-step principles and harm reduction
  • Understand and endorse SAMHSA Ten Principles of Recovery
  • Prior case management experience in a medical or community setting preferred
  • Experience working with people who have substance use and behavioral health diagnoses strongly preferred
  • Bilingual strongly preferred
  • Excellent interpersonal skills
  • A demonstrated ability to adeptly negotiate recovery systems and culture in order to most effectively serve patients all along the motivational continuum
  • Ability to be an effective team member and remain accountable to team goals while working frequently off-site
  • Flexibility, adaptability, independence, and ability to multitask required
  • Experience working with ethnically, culturally and racially diverse co-workers and clients preferred
  • Experience with LGBTQ population preferred
  • Ability to work harmoniously with diverse groups of individuals required
