Helps consumers develop a realistic service plan and collaborates to achieve measureable objectives. Service Coordination and Documentation Refers and links consumers to Supported Employment and the Division of Vocational Rehabilitation Facilitates coordination and communication among staff around pre-vocational needs. Prepares and maintains accurate up-to- date case records in accordance with agency standards, accreditation and funding source requirements Together with Program Supervisor assesses effectiveness of programming in meeting prevocational needs. Participates in Quality Improvement evaluating the effectiveness of prevocational services Other duties as assigned.   Position Requirements Qualifications: Bachelors Degree in a human services field Certfication in Pre-Vocational Services with Two years experience in providing mental health services or Experience in providing pre-vocational services. ~cb   Exempt/Non-Exempt Exempt   Weekly Work Hours 35   Proposed Salary $35K   Schedule M-F, 8:30am to 4:00pm  This position is currently not accepting applications.
