We are seeking mission focused professionals to serve as a part time Case Coordinator responsible for providing in-home support services for foster parents and children.

As a Case Coordinator you would:

  • Create your own schedule based on your caseload and availability of your assigned families. This includes potentially working until 8pm several nights a month, based on the needs of the families.
  • Provide in-home, direct services to families and children in your caseload. Generally, caseloads consist of 7-9 clients with whom you visit and support 2-3 times a month.
  • Potentially be asked to assist in our rotating on-call schedule, as needed.
  • Create documentation for all services you provide in our Electronic Record System. Documentation includes Assessments, Diagnosis, Treatment Plans and Progress Notes, and Monthly reports. Overall documentation time is about 20%.
  • Develop and maintain consultative and liaison relationships with DCBS and other members of the child’s treatment team (biological parents, school and SAFY team members)
  • Travel to families near your home, in and around Lexington. Overall hours being worked outside of the office are estimated to be around 40-60%, which includes traveling to and from visits and time spend at the home conducting a home visit.
Position Requirements

What you bring to this role:

  • Bachelor’s degree in social work, counseling, psychology, sociology, or child & family studies is required.
  • New graduates welcome! 2+ years of experience working with families and children, child welfare, and in-home therapies. Practicum hours can count toward experience.
  • Passion for serving youth, strong time management skills and working as part of a team.
