We are seeking mission focused, full time LIFE Coach responsible for preparing youth in care to begin the transition from out of home care to living interdependently, and will coach youth who have aged out of care to successfully transition to interdependent living in our Merrillville division.

As a LIFE Coach, you will:

  • Work with DCS and collaborative care youth to address immediate needs for housing, food, clothing and other basic needs immediately upon referral to collaborative care. Primary needs areas include: safety, housing, education, and employment. Assess existing life skills using the Casey Life Skills Assessment.
  • Develop youth driven goals and Learning Plan (within 30-45 days) for developing the skills and resources necessary for successful transition to independence. Encourage youth in the achievement of the goals within the Plan. Integrate long term plans for safety, housing education, employment and financial management into Learning Plan.
  • Work with transition team to ensure that all youth in the program have transition plans with appropriate supports in place for the plans.
  • Meet with youth as required in the DCS contract and as determined in CFTM, and work to develop rapport with them. Assess the strengths and needs of youth to form the basis of strength-based planning. Empower youth by addressing trauma-related issues as they arise in the transition process.
  • Documentation: It is approximated that one will have 8-10 hours of documentation a week to complete which is due within 72 hours of the service.
  • Standard work hours are Mon-Fri (some Sat.). Time frame is flexible and depends on session (summer vs school year) but staff must be available to youth (typically 3-8 pm) for home visits a few nights a week.
  • Travel includes both city and rural areas. Counties include: Lake, Porter, Laport, Jasper, Stark and Pulaki. You can expect to be on the road approximately 10-15 hours per week. Overall hours being worked outside of the office is estimated to be around 28-40 hours/week which includes traveling to and from visits and time spend at the home conducting a home visit.
  • On-call: Emergency calls from youth can come in 24/7. *This is not a regular event.
 
Position Requirements
  • Bachelor's degree in the field of social work, counseling, psychology, sociology, child & family studies, or other related field of human services.
  • 1-3 years of experience in working with youth/young people highly preferred.
