About the Organization CP Rochester and Happiness House are private nonprofit health, educational, residential and human service organizations that have been serving communities in the Finger Lakes Region since 1969 and the Greater Rochester Area since 1946. Today with sites in Rochester, Canandaigua, Geneva, Gorham, Midlakes and Waterloo, our agencies offer an array of educational, therapeutic, recreational, residential and family support programs and services for children and adults with and without disabilities. On average, our agencies serve 3,650 families each year and employ approximately 600 people.


Our staff consists of dedicated and knowledgeable professionals and clinicians with expertise in the areas of Special Education, Early Childhood Education, Physical, Occupational, Speech and Music Therapies, Psychology, Service Coordination, Nursing, Dental Services, and Developmental Disabilities.


Committed to serving individuals with autism, traumatic brain injury, neurological impairments, as well as a wide variety of developmental disabilities such as cerebral palsy and Down syndrome, our staff partners with individuals and their families to support them in meeting their goals.

Description

Qualifications:

  • Master of Social Work, Master of Psychology or Certified Rehabilitation Counselor.
  • Must have a minimum of two (2) years experience providing adjustment related counseling to individuals with physical, developmental or psychiatric disabilities.
  • Valid NYS driver's license and dependable transportation.

Responsibilities include, but are not limited to:

  • Imparts agency philosophy of 'Equal Opportunity, Independence and Realization of Individual Potential' to clients, families, program staff and community resources.
  • Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
  • Develops a plan of service to assist an individual to more effectively manage and overcome the difficulties and stresses encountered by surviving a brain injury.
  • Provides counseling and service to an individual and/or a family member, or anyone that may have a significant role in the support of the individual with a brain injury.
  • Provides service to a consumer or their family in the individual's home, community, or the provider's office based on the comfort and needs of the consumer and their family.
  • Participates in team meetings as scheduled by the consumer and their service coordinator, providing feedback and recommendations for the consumer's overall plan of service.
  • Acts as a role model for the consumers by interacting with others in a professional and respectful manner, dressing appropriately, and using good personal hygiene.
  • Maintains necessary documentation as required and per agency policies.
  • Communicates regularly with agency personnel to provide opportunities for exchange of information and discussion of concerns. Provides feedback to Program Director regarding consumer concerns.
  • Manages crisis situations per agency policies and procedures.
  • Participates in conferences, workshops and in-service training as requested to enhance job skills and professional development as determined by the Supervisor.
  • Adheres to the agency's policies and procedures as well as department guidelines.
  • Performs other duties at the discretion of the Program Director or his/her designee.
