Focusing on permanency, safety, and well-being and needs within the context of the family and community, we build on each individual's strengths to achieve positive, sustainable outcomes.  

The Bridges Liaison is responsible for the case management functions of a caseload for youth/young adults in the Bridges program.

Essential Responsibilities:

  • Help youth develop a treatment plan/goals.
  • Meet with youth face to face at least once a month.
  • Review treatment plan with youth every 90 days to acknowledge their achievements and plan their next steps.
  • Help youth maintain program eligibility if their circumstances change.
  • Help youth identify individuals and resources that can be permanent connections even after they've completed the program.
  • Teach youth skills like money management, how to do laundry, how to grocery shop, etc.
  • Help with college or a vocational program enrollment.
  • Assist with employment.
  • Help youth find a doctor or access other physical or mental health care.
  • Connect youth with community resources to help them become more self-sufficient.
  • Provide guidance and be a positive adult supporter.
Position Requirements

Education: Completion of a post-secondary program (may be an Associate’s degree or related certification) in social work, human services, behavioral health or related field is required, BA preferred.

Licensure: LSW or LPC preferred

Experience: Two years of relevant experience working with families and/or children in the child welfare system; or possess a bachelor’s degree in social work, human services, behavioral health or a related field.

Skill Set:

  • Excellent communication skills (oral and written)
  • Resolution skills to demonstrate effective problem solving
  • Strong organizational skills to prioritize workload
  • Attention to detail and accuracy and ability to multitask
  • Operate a computer with Microsoft Office, other software, agency databases and online systems
  • Demonstrated skill of writing treatment plans, case notes, etc.

Additional Requirements:

  • Acceptable driving record and auto insurance in order to meet the travel requirements
  • Ability to work flexible hours daily/weekly to fulfill position duties. Flexible work hours may be long work days as well as weekend work hours
  • May require transporting youth if needed
