The VA REACH Lead Counselor-Crisis Therapeutic Home provides both the standard role of Therapeutic Counselor with additional shared responsibilities with the CTH Director. This position will provide support to guests with co-occurring intellectual/developmental disabilities and mental or behavioral health needs who are temporarily accessing crisis or planned therapeutic visits within the REACH Crisis Therapeutic Home. This staff participates in facilitating daily program and individualized activities and support for habilitative and/or rehabilitative activities, therapeutic interventions, personal care/assistance, health care, and other activities. This position participates as part of comprehensive REACH services that collaborate with guest’s primary provider and clinical home, community crisis services, family, and others.

Essential Job Functions:

  1. Assists guests with daily activities, personal care/assistance, behavioral health needs, and physical health, wellness, community engagement and leisure activities.
  2. Orients guests and their caregivers (if applicable) to the CTH, program policies, rules and activities.
  3. Establishes rapport with guests with intellectual/developmental disabilities and challenging behaviors, while maintaining respect for each guest’s dignity, individuality and right to confidentiality.
  4. Maintains professional boundaries with guests and follows ethical and licensing guidelines.
  5. Monitors guests for frequency and severity of mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with individualized crisis education and prevention plans, behavioral support plan (if applicable), overall service plan (e.g. ISP, PCP, POC).
  6. Participates in development and implementation of guests’ crisis education and prevention plan.
  7. Plans, structures and prompts social and leisure time activities, including activities that build skills for behavior management, stress management, communication, adaptive living, ADL’s and IADL’s.
  8. Serves as a positive representative for agency with other professionals, families and the community.
  9. Reports incidents as required by agency policy as well as local state and federal laws.
  10. Maintains timely documentation, progress notes, behavior/symptom tracking, and other data, including incident and accident reports.
  11. Responds to individual safety needs, including reporting significant changes in behavior and health.
  12. Collaborates with the CTH Director, Assistant CTH Director/RN, Medical Director, Clinical Director and other staff with clinical expertise on presentation of needs, behavioral tracking, attempted interventions, plan of care and discharge planning.
  13. Communicates with other staff and supervisor on progress, needs, concerns and changes.
  14. Performs housekeeping duties as assigned, including meal preparation in accordance with individual dietary needs and preferences.
  15. Supports CTH Director and Assistant Director/RN with interviewing, hiring, and orienting recently hired CTH Counselors.
  16. Creates staffing schedule on a monthly basis and provides ongoing monitoring to ensure that appropriate supports and levels of supervision are maintained.
  17. Adheres to program standards and policies as well as local, state and federal regulations.
  18. Assists with discharge planning.
  19. Other duties as assigned.
  20. Follows all ESUCP procedures as appropriate to position and employment.


Supervisory Responsibility:
This position has Lead Shift responsibility, including providing oversight of the REACH Therapeutic Home Counselors, and direct support to the CTH Director. The position provides administrative oversight of specific program operations in the absence of the CTH Director. This position shares the function with the CTH Director of acting as liaison between the REACH Clinical team and TH team.

 
Position Requirements
  • Bachelor’s degree from an accredited college or university in a human services field including, but not limited to, sociology, social work, special education, rehabilitation counseling, or psychology; or a bachelor’s degree from an accredited college or university in another field in addition to an advanced degree in a human services field and at least 1 year of direct clinical experience with ID/DD/MH population.
  • Experience with crisis prevention, stabilization, and/or intervention services required.
  • Maintain valid, state appropriate driver’s license and insurance for personal and agency vehicles utilized for work;
  • Complete within specified time period the trainings as required by agency and service model.
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
  • Must demonstrate capacity to effectively communicate verbally and in written form.
