Description

CLASS SUMMARY

Directly supervises and monitors functional community living, home-living, develops academic and vocational training programs, provides direct care and case management for the adults with intellectual disabilities and who may have other disabilities including physical, cognitive or mental disorders. This class completes administrative duties assigned by the Residential Services Supervisor.

DISTINGUSHING ELEMENTS OF THE CLASS:

  1. IMPACT: Immediate health and welfare of client is directly affected by work of this class. Activities of incumbent can promote successful community integration for the client and assist in preventing unnecessary institutionalization.
  2. LATITUDE: Day-to-day work is performed independently with regular consultation and frequent follow-up from the Program Supervisor. Work is guided by the Individual Program Plan, program policies, ICF/MR regulations and guidelines and Program Schedule.
  3. RESPONSIBILITY FOR THE WORK OF OTHERS: None
  4. INTERPERSONAL CONTACTS: Continuous contact with clients and other staff in planning and providing the clients services. Regular contact with family members and members of other agencies providing services to clients, such as social workers, employers, physicians and other service providers.
  5. Difficulty: Appropriately relating to a variety of client medical challenges, assistive needs, and personality types requires use of judgment and discretion in emergency situations resulting from client medical or behavior problems involve timely and responsible decision making. Incumbent must deal with a variety of other human services officials implementing individual program plans.
  6. COMMUNICATIONS: Work involves regular oral and written communication with Program Supervisor, Program Manager, clients, employers, service providers, and families. Work also requires incumbent to prepare written documentation such as reports, individual program plans, functional skills plan objectives, business communications, client progress notes, etc. Public relations activities are frequently involved.
  7. EQUIPMENT/FACILITIES RESPONSIBILITY: Incumbent will be responsible for equipment at center based and residential sites such as vehicles, adaptive equipment, TV’s, Microwave ovens, stoves, dishwashers, recreation equipment, lifts and associated furnishings and/or equipment.
  8. WORKING CONDITIONS: Working is performed in residential environment. Incumbent works at the facility or job site during assigned shift. Frequent outside activities expose incumbent to elements of nature. Some clients can be abusive.
  9. KNOWLEDGE< SKILL AND ABILITIES (KSA’S):
    • Good knowledge of mental retardation, autism, and other developmental disabilities.
    • Knowledge of ICF/MR Regulations and guidelines.
    • Demonstrated skill in working with adults with disabilities.
    • Very good oral and written communication skills.
    • Ability to establish and maintain effective working relationships with clients, families, employers, and other service provider staff.
  10. LICENSURE: Valid VA driver’s license.
  11. PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS:
    • Develop, inservice and implement training goals for home living, community living, academic and vocational training for assigned consumer caseload with specified time frame.
    • Monitor training of goal plans and program books to assure appropriate procedures, continue functionality and notification of completion of goals. Revise goal plans as indicated by consumer need with QMRP approval.
    • Provide accurate data for assigned caseload within specified time frame.
    • Conduct ongoing functional duties to insure appropriate methodology and implementation.
    • Attend and request core team meetings for assigned caseload to discuss revisions, completion or implementation of goal training.
    • Assure materials are available and appropriate for goal training and integrative activities.
    • Provide inservice training to ensure consumers/staff are accountable to program rules, policy and procedures.
    • Coordinate staffing assignments in conjunction with the Residential Supervisor; make changes as necessary on daily basis according to consumer needs.
    • Regularly review and comply with all ICF certification and DSH licensure standards. Assure compliance with local State and Federal law, including OSHA and HNNCSB Health and Safety regulations and standards.
    • Monitor program building areas for compliance with OSHA standards. Insure quarterly audit is conducted and submitted to Residential Supervisor.
    • Meets work schedule in a reliable and accountable manner.
    • Interacts with consumers in a positive and therapeutic manner.
    • Provide new employee inservicing on training method as scheduled by HR Department or Residential Services Manager.
    • Complies with all HNNCSB policies and procedures.
  12. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a personal computer, calculator, copier, and fax machine. Some running after clients, lifting from 100-300 lbs, walking up and down stairs, pushing and pulling wheelchairs 275-350 lbs, assisting clients on and off vans, and some kneeling and crawling.
  13. DRIVING REQUIREMENTS: Valid Driver’s License. Transports clients as necessary.
