Rapidly expanding local franchise of Right at Home, a major international home care company, seeks a Care Manager to help ensure that our clients continue to receive the best possible home care available in northwest Indiana. The Care Manager will report directly to the President/Owner and Office Manager of the company with three primary responsibilities: (1) conduct comprehensive in-home assessments with clients and prospective clients; (2) assist with training caregivers, conducting supervisory visits, and providing quality assurance; and (3) provide back-up on call coverage on the weekends and periodically assist with scheduling caregivers for appropriate shifts.

The ideal candidate for the Care Manager position is an enthusiastic and dynamic problem-solver who is proactive, highly organized, detail-oriented, an effective communicator, and has the ability to thrive in a fast-paced environment. This position will serve as a key liaison between clients, office staff, and caregivers. A CNA certification or higher (e.g., LPN or RN) is required for this position.

RESPONSIBILITIES

The primary responsibilities of the Care Manager are to facilitate all aspects of client care including development and implementation of customized care plans and ongoing assessment of client status, conduct supervisory visits, implement in-house caregiver training programs to meet the ongoing needs of our clients, and light scheduling duties including back-up on-call support after hours. The Care Manager also supports the supervision of caregivers to maintain the “gold standard” of home care in northwest Indiana that Right at Home has come to represent.

Assessments

  • Conduct comprehensive in-home assessments with prospective clients and formulate an individualized client care plan that incorporates analysis of assessment data.
  • Ensure client care standards are in compliance with state regulations and agency policies by using specialized knowledge and judgment.

Quality Assurance / Training

  • Monitor and improve caregiver job performance through in-home supervisory visits and performance evaluations.
  • Supervise case management activities for individual clients.
  • Develop and conduct in-house caregiver training programs in order to demonstrate and institutionalize Right at Home’s “gold standard” of care.
  • Develop relationships with power partners to provide additional caregiver training on specialized topics including, but not limited to, dementia and Alzheimer’s, hospice care, proper use of medical equipment, and other care needs.
  • Notify management of quality assurance issues, and document and resolve grievances.

Scheduling

  • Serve as a back-up to weekend on-call answering service.
  • Verify answering service is responding to incoming after-hours calls in a courteous and timely manner and promptly handling issues.
  • Assist answering service with scheduling caregivers to fill open shifts if/when the preassigned caregivers need to call-off due to emergencies.
  • Communicate with clients and caregivers to ensure needs are met.
  • Light scheduling duties during normal business hours.

Other Responsibilities

  • Maintain compliance to all agency non-medical guidelines, administrative policies, procedures, and regulatory requirements.
  • Promote a positive image of Right at Home within the office and in the community.
  • Maintain client confidentiality at all times.
  • Other duties as assigned.

 
Position Requirements

SKILLS AND ABILITIES

  • Self-starter able to work independently using sound judgment.
  • Ability to multi-task and work in a fast-paced environment.
  • Highly organized, able to prioritize and change direction as needed.
  • Strong computer skills and comfortable with technology – proficient with Microsoft office and familiar with or able to learn and use ClearCare software system.
  • Possess a strong desire to serve others -- former experience caregiving and/or at a home care or home health agency is a plus.
  • Great energy, empathy, and a willingness to do what it takes to get the job done.
  • Team player with an “all hands on deck” attitude.

REQUIREMENTS

  • 2+ years of experience as a CNA, LPN, or RN.
  • Experience with case management, preferably within in-home care, home health, or a medical office.
  • High school diploma required, college degree preferred.
  • Valid driver's license, current auto insurance, and reliable transportation.
  • Travel within northwest Indiana.
