Director Care Manager - Hub & Spoke 


What are we looking for in a Director Care Manager?

The Director Care Manager is responsible for providing leadership, education, support, and outreach to the Hub & Spoke Network. As this is a new treatment model for us, the Director Care Manager will need to develop and establish best practices for referring, coordinating, and managing the client experience and expediting access to care across the various treatment providers. The Director Care Manager may also participate in direct service provision as needed to ensure efficient access to care within the program.

The Hub & Spoke model is a network of community providers that work together to provide integrated care for individuals receiving medication assisted treatment for opioid use disorders. This integrated approach assists clients with multiple facets of their recovery, regardless of which service provider forms the initial connection.

Director Care Manager duties may include:

  • supervising and providing direction to all members of the Hub & Spoke team
  • developing network of community substance use providers
  • providing management, outreach, education, and training to Hub& Spoke team and other multidisciplinary treatment teams
  • assisting leadership with developing & establishing best practices for managing the client experience
  • communicating with care team members, community members, psychiatrists, and primary care providers to coordinate services for Hub & Spoke clients
  • facilitating referrals and serving as the central contact for clients, specialists, and community providers as needed
  • assists with direct case management services, appointment coordination, and prescription management as needed
Position Requirements


  • Bachelor's degree in social work, psychology, or related field OR Registered Nurse or Licensed Practical Nurse with one year of experience OR Chemical Dependency Professional with one year of experience
  • Prior administrative and supervisory experience required
