About the Organization KMHS is Kitsap's nonprofit, designated community mental health provider for adults with acute and/or chronic serious mental illnesses, and children with serious emotional disturbances and their families. During 2016, KMHS provided services to more than 6,800 children and adults.

The Peer Bridger/Discharge Planner supports the multidisciplinary team by providing and tracking discharge-planning services that are complex, multi system, and specific for the people we serve on the inpatient units. Using lived experience, the Peer Bridger/Discharge Planner focuses on providing hope and discussing practicality with the individuals supported. Having made use of various resources, the peer bridger/discharge planner assists the discharging person with navigation of external resources.


  • Coordinate and facilitate timely implementation of discharge plans for Enrollees. Develop and coordinate discharge plans that are: complex, multi system, mixed funding, and specific to Enrollees that would otherwise be transferred to the state hospitals.
  • Track the Enrollees progress upon discharge for no less than thirty (30) days after discharge from the facility. Arrange follow-up care as appropriate.
  • Maintain effective communication with physicians, nursing staff, individuals served, families and other related to discharge planning.
  • Coordinate with social services personnel to provide services as indicated by treatment team.
  • Document findings, discharge plans and action taken on medical record according to -KMHS guidelines; prepare reports and maintain records as required.
  • Maintains the clinical information of individuals served by the unit in a confidential manner.
  • Attends orientations, staff meetings and keeps all yearly mandatory trainings up-to-date as required by the unit.
  • Supports the Mental Health Professionals in completing case management tasks associated with discharge such as arranging for shelter beds, and follow up appointments.
  • Participate in treatment team meetings 3 times per week at the unit.
  • Track the total number of all discharges from the unit and differentiate between those that were deemed complex and those that were deemed standard.
Position Requirements

Current WA State Peer Counseling certificate or complete certification within six months of employment. Preference for lived experience in an inpatient setting. Ability to gain licensure as an Agency Affiliated Counselor (or other applicable counselor profession) in Washington State, valid Washington State Driver License and Food Handler card. High School Diploma or GED required. A bachelor’s degree in a counseling-related field is preferred.
