Diversity at our agency means fostering a workplace in which individual differences are recognized, appreciated, respected and responded to in ways that fully develop and utilize each person's talent and strengths.   Position Per Diem Peer Specialist - NYC   Full-Time/Part-Time Per Diem   Location East New York Ave, Brooklyn NY   Salary   Days/Hours per diem   Description Provide strength-based rehabilitative and recovery oriented services as part of peer-to-peer evidence-based practices under HCBS; Be willing to share personal experiences to foster sense of hope and recovery; Provide face-to-face individual services in a clients home or community following the HCBS protocols, documenting all services in the Electronic Health Record (EHR) using collaborative documentation; Carry a caseload of four to eight clients; Ensure coordination of services through collaboration and consultation with other staff, providers and identified family and/or friends of the tenant/ resident; Assist tenants/ residents with entitlements and concrete services; working together and advocating on their behalf via telephone and written correspondence; Attend and participate in weekly supervision, staff meetings and periodic in-service trainings, including developing competencies in evidence-based practices; Document time and effort and tasks during working hours as required for billing; Other duties as assigned.   Position Requirements Qualifications: ·High School Diploma or GED ·OMH or OASAS approved peer certification ·Two years life experience with behavioral health issues ·Experience with advocacy, outreach and engagement, self-help, recovery and other local support systems ·Knowledge of Evidenced Based Practices (Motivational Interviewing, Harm Reduction) Bi-lingual Spanish/ English preferred Experience with Electronic Health Record (EHR ) a plus   EOE Statement We are an equal employment opportunity employer.
