Outreach and Referral Coordinator

We are currently seeking an Outreach and Referral Coordinator to support the ACTT program in Statesville, NC. Master's degree and licensures are required

Position Summary:

The Outreach and Referral Coordinator serves as the liaison and entry point for referral sources and stakeholders who interact with ACT Teams in their assigned location. The Coordinator assists with referrals, networks with community stakeholders, and markets the ACTT program to referral sources. The Coordinator conducts screenings for services and comprehensive clinical assessments as needed.

Essential Job Functions:

  • Completes initial screenings for the assigned teams.
  • Markets the ACTT Program and Easterseals UCP (ESUCP) by identifying referral sources and important stakeholders and creating strategies to actively engage them.
  • Makes weekly contact with new referral sources by email, phone, and/or drop-in.
  • Seeks opportunities to educate community stakeholders about ACTT services and the SPMI population through presentations and providing educational materials.
  • Attends community meetings as a representative of ESUCP. These meetings include, but are not limited to, adult community collaboratives, Partners Enhanced Services Collaborative, and Provider Council meetings.
  • Pursues opportunities to attend community events in order to market ACTT services and organizes promotional table/booths at community events, coordinating with other staff to ensure prompt delivery of materials and merchandise.
  • Completes Comprehensive Clinical Assessments as needed/requested for assigned mental health programs/cost centers.
  • Ensures all assessment activities are properly entered in to electronic health record to ensure accurate and timely billing.
  • Completes all necessary and assigned documentation (e.g. screening form, service notes).
  • Other duties as assigned by Program Manager.
Position Requirements

Minimum Qualifications:

  • Masters degree in a Human Service related field from an accredited institution recognized by the US Department of Education and Qualification as a Qualified Professional according to 10A NCAC 27G.0104 required.
  • Licensure as a LPC, LCSW, LMFT, or LPA required.
  • Minimum of one year documented experience working with the adult mental health population required.
  • Must demonstrate high moral and ethical character.
  • Maintain valid driver’s license and insurance for personal vehicles utilized for work
  • Complete all trainings as required by agency and service model.
