Function as a member of a multidisciplinary team while demonstrating the clinical skill and experience to plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients. Assist PACT Team to provide integrated substance use services and mental health treatment for KMHS clients to include: collaborative treatment planning, individual, family, and group and perform all duties necessary to comply with relevant Kitsap County Chemical Dependency Treatment Contracts and all other regulatory agencies. A minimum of 75% of all services delivered in the community via assertive outreach.
RESPONSIBILITIES AND DUTIES:
Current licensure as a Chemical Dependency Professional (CDP) or Chemical Dependency Professional Trainee (CDPT) in WA State preferred, a Associate’s Degree in a social services discipline; significant experience working with clients with co-occurring mental illness and substance abuse disorders; a valid WA State Driver License and reliable, insured transportation are required.
