During 2016, KMHS provided services to more than 6,800 children and adults.

Description

PURPOSE:
This position is part of a newly funded program, which will be facilitated in collaboration with Kitsap Public Health District.  This collective impact project, which is founded on principles of harm reduction and collaboration, will help some of the most vulnerable residents of Bremerton and Central Kitsap, who are experiencing (or are at risk of) mental illness, chemical dependency, physical illnesses, and homelessness by improving access to the care they need.  Using a multi-disciplinary, mobile outreach team that includes intensive care coordination services, this project will provide the critical engagement and connective tissue to link these residents with desperately needed services that will ultimately reduce the severity of behavioral health issues, serious physical health complications, and homelessness.  This will, in turn, reduce inappropriate or unnecessary utilization of costly services.  The Behavioral Health Professional will participate on the Crisis and Care Coordination Team as the liaison and expert for Behavioral Health; while this position involves some direct care, it is predominately focused on intensive care coordination of services for highly vulnerable clients.  Work is performed primarily in the field and community settings.

RESPONSIBILITIES AND DUTIES:

  • Conducts intake/assessment interviews with clients having complex social, behavioral, mental, and physical health issues who are referred to the team by various partner agencies.
  • Makes field visits and telephone calls to assess client needs, link client to needed resources, coordinate care, and arrange for needed supportive services.
  • Participates in team care plan development and care coordination conferences.
  • Serves as the team content expert on behavioral health.
  • Works collaboratively with internal and external partners including:  social service agencies, the court system, Crisis Response Team, Adult Inpatient Unit, KMHS Medical providers, the medical community, The Kitsap Recovery Center, KCR, EMS, Harrison Hospital, law enforcement, criminal justice and other treatment providers to assist clients in their recovery process and to ensure the continuity and quality of care.
  • Advises on care planning development, specifically promoting chemical dependency, recovery and effective treatment for mental illness.
  • Provides effective and efficient clinical services, using best practice standards, to adults who are participating with the Crisis and Coordination Team.
  • Provides Mental Health and Co-Occurring Disorder treatment services including outreach, as clinically indicated, to clients, in the Crisis and Care Coordination Team.
  • Provides direct services for clients at risk for and with co-occurring disorders utilizing recommended Co-Occurring Disorder treatment practices. Services may include brief intervention, group therapy, psycho-educational workshops, skill building groups, and crisis intervention.
  • Provides linkages to KMHS service providers and other community resources and agencies as appropriate and help the client maintain linkages.
  • Utilizes motivational interviewing, stages of change theory, family and system focused interventions, stage-wise treatment strategies to intervene and motivate clients.
  • Conducts behavioral health screenings to aid in treatment planning, utilizing clinically appropriate instruments.
  • Demonstrates a working knowledge of psychotropic medications and their side effects and contraindications with drugs of abuse and report any relevant observations to medical personnel.
  • Pursue professional growth and development by seeking out learning opportunities and integrating new learning into daily work practice. Maintain working knowledge of Evidence Based Practices, current Co-Occurring Disorder treatment recommendations and trends in community mental health, including health care reform.
  • Complete and maintain all required appropriate clinical and administrative records in a timely manner.
  • Provide crisis intervention services when necessary.
 
Position Requirements

Master’s degree in psychology, social services, or a related field preferred.  Minimum of two years’ experience working with clients with mental illness preferred.  Must be passionate about working with people who face complex social and health issues, including homelessness, chemical dependency, mental illness, and chronic disease.  Must fully embrace and be willing to advocate for harm reduction strategies.  Prefer experience in co-occurring disorders treatment, individual and group therapy, and substance abuse treatment.  Possess and maintain valid driver's license with an acceptable motor vehicle report and reliable, insured transportation
