Clinical Manager for Home Health Agency 

Description

The RN Clinical Manager for Home Health Agency, under the general supervision of the Administrator of Home Health and Private Duty Services, provides and directs coordinated, comprehensive clinical services to individuals and families in the Lakeview Village and surrounding community setting. Emphasis is placed on meeting the needs of persons in their homes, upon promotion and maintenance of health and on early detection of complications to prevent return to the hospital.

  • Assist Intake Coordinator with coordinating the admissions. Obtain orders, authorization from insurance company, verifying physician, and face to face documents.
  • Perform quarterly chart reviews to assure appropriate utilization of services.
  • Assure the accurate and timely completion of the OASIS are done by the Case Mangers//Field Nurses. Assist with scrubbing, and submission to CMS.
  • Assure that patient’s plan of care are developed, implemented, and updated according to regulations
  • Develop, implement, and coordinate with patient(s), family, primary physician, and the home health team individualized and comprehensive care plan for the patient(s) with expected outcomes that are measurable and realistic.
  • Review accuracy of 485 and submit according to regulations
  • Assure appropriate physician orders are obtained and followed, according to regulations
  • Maintain appropriate communication with the physician as required throughout the course of care.
  • Complete the finals for billing
  • Track and manage the face to face visit requirements
  • Perform start of care visits, as needed based off need/staffing
  • Perform QA checks on medical records, as requested
  • Participate in planning for orientation of new employees
  • Conduct yearly skills check off for employees
  • Back up the Administrator/DON in their absence.
  • Rotate call on weekends and during the week as directed by the Administrator
  • Other duties as assigned
 
Position Requirements
  • Graduate of an accredited registered nurse program.
  • Current and unrestricted RN license in the State of Kansas.
  • A minimum of 3 years supervisory experience in home health required.
  • Current valid state driver’s license and adequate transportation.
  • Standing, bending, lifting, stooping and performing other work requiring light physical exertion (up to 30 lbs.) on a frequent basis; continuous walking; moderate physical exertion (30-50 lbs) on an occasional basis and heavy physical exertion (over 50 lbs) on an intermittent basis (not a routine part of the job).
  • May be exposed to unpleasant working conditions or undesirable elements that may involve exposure to a variety of components that are potentially hazardous or harmful (OSHA Exposure Category #1: High risk to exposure to blood and body fluids.).
  • Requires advanced knowledge of Medicare, Medicaid, Managed Care, other 3rd Party Insurance plans and State/Federal regulations as they apply to Home Health.
  • Efficient with computer skills and electronic medical record systems.
  • Must be 21 years of age
 
Full-Time/Part-Time Full-Time  
Shift Monday-Friday, 8am-5pm; weekend on-call requirements.
