Description

PURPOSE:
Function as a team member while demonstrating the clinical skill and experience to assess, develop treatment plan, coordinate, and provide services to Substance Use Disorder (SUD) clients. To provide Medication Assisted Treatment (MAT) services for Kitsap Mental Health Services (KMHS) clients to include assessment, collaborate in treatment, individual, family and group therapy, education and perform all duties necessary to comply with relevant contracts and all other regulatory agencies;. To coordinate and provide delivery of MAT counseling services.

RESPONSIBILITIES AND DUTIES:

  • Monitor and respond to the dedicated MAT phone line in order to coordinate admission and assessment within 48 hours.
  • Coordinate initial medical provider visit for the MAT program.
  • Conduct SUD assessment for adults to determine diagnostic and the appropriateness of admission and level of care based on American Society of Addiction Medicine (ASAM) Patient Placement Criteria 2.
  • Provide all functions of outpatient counseling to include, but not limited to outreach calls, assessment, group therapy, individual counseling, relapse prevention, family therapy, case management, and discharge planning.
  • Utilize motivational interviewing methods based on stages of change, trauma informed care, and stage-wise treatment strategies to intervene and motivate SUD and Mental Health (MH) clients for change.
  • Provide consultation services and training to Care Team and other program staff as required to assist in integration of MAT SUD services in KMHS.
  • Demonstrate ability to establish therapeutic relationship with clients while maintaining professional boundaries.
  • Routinely identify and evaluate physical risk factors to self and others. Consult with supervisor, Crisis Response Team and law enforcement as appropriate. Demonstrate ability to provide crisis intervention services during assessment and treatment process. Be proficient in recognizing and facilitating care when encountering sign and symptoms of use and withdrawal and/or mental health decompensation.
  • Demonstrate working knowledge of MAT/psychotropic medications their side effects and contraindications with substances of abuse and report any relevant information to care team medical providers.
  • Monitor clients for compliance with court-ordered requirements such as Least Restrictive Alternatives (LRAs) and substance related mandated clients. Provide appropriate legal documents and monthly reports as required.
  • Perform random Urine Analysis tests substance abuse when clinically indicated.
  • Use collaborative documentation and document all services thoroughly and accurately. Submit all documentations in a timely manner per agency policies and procedures, contractual and Washington Administrative Codes.
  • Demonstrate ability to work efficiently and effectively in a managed care environment, organizing workload by prioritizing and managing multiple priorities
  • In addition to the above, any responsibilities appropriate to the position and not specially listed to the job description.
 
Position Requirements

Current certification as a Chemical Dependency Professional (CDP) in the State of Washington. Significant experience working with SUD clients. Associate of Art Degree (AA). A Bachelor’s /Master’s Degree in social services discipline is preferred.
