Description

PURPOSE:
Provide discharge planning for older adult from Adult Inpatient Unit (AIU) and Western State Hospital clients, outpatient assessment, psycho-therapy, outreach services, consultation and education to community health service providers. Provide outpatient, outreach and day treatment services.

RESPONSIBILITIES AND DUTIES:

  • Provide individual and group assessments, intakes and mental health therapy through multi-disciplinary approaches to the elderly.
  • Provide direct and indirect mental health services to the elderly who reside in long-term care facilities (i.e., adult foster homes, nursing homes and boarding homes) to include outreach, short and long-term individual and group therapy, advocacy and consultation. Assist in maintaining an ongoing professional relationship with community agencies. Serve the elderly so as to promote coordination and service delivery.
  • Assist with the development and facilitation of therapeutic groups as needed.
  • Identify program needs and assist in enhancing and developing programs.
  • Provide direction as needed to Treatment Coordinators regarding treatment plans as assigned.
  • Complete and submit relevant client documentation in an accurate and timely manner in accordance with agency requirements.
  • Demonstrate knowledge of, or be willing to work adequately with, clients with dual diagnoses.
  • Provide mental health support for clients who are being treated under the Expanded Community Services (ECS) contract. This includes screening, assessment, outreach, coordination of care and staff training as needed.
  • In addition to the above, any other responsibilities appropriate to the position and not specifically listed in the job description.

 
Position Requirements

Master's degree in social sciences and qualification as a mental health professional; two years experience with older adults or Registered Nurse with older adult experience; OBRA PASARR training and dual diagnosis training preferred. Must possess and maintain valid driver's license with an acceptable motor vehicle report, and reliable personal transportation with business-rated insurance.
