General Job Description The Transition Coordinator will serve as a point of reference for students, parents, faculty in preparing student for and linking students to post- secondary opportunities

Academic Requirements;

Requires a bachelor's degree in special education, guidance, counseling or related field. Strong transition planning a plus. Ability or potential to develop collaborative community relationships. Experience working with student with emotional disturbance, autism spectrum and moderate cognitive disabilities.

Partial list of job resonsibilities to inclue:

  1. develop transition plans
  2. provide opportunities for service learning
  3. make referrals to appropriate adult agencies
  4. develop internship opportunities with local businesses
  5. provide support and training to staff supervising students at work sites
  6. provide support and training to employers providing internship opportunities
