Description

JOB SUMMARY: Provide consumer diagnostic assessments and admission services for Partial Care Program.

RESPONSIBILITIES:

  • Orient potential consumers to Catholic Charities Partial Care services.
  • Conduct diagnostic and level of care assessments.
  • Complete admissions paperwork including authorizations and initial service plans.
  • Complete an initial service plan during the intake process.
  • Complete a written comprehensive assessment for each consumer during intake process. The written comprehensive assessment will be completed within one month after acceptance to the program and prior to development of the individualized recovery plan.
  • Write progress notes at least weekly per DMH guidelines. This includes the following: Completion of the daily rating scale with consumer self-report, Psychosocial Assessment which will be completed within the first six weeks of admission; Individualized Recovery Plan will be formulated and implemented at the completion of the comprehensive assessment but no later than six weeks from admission; IRP will be reviewed and revised within three months of its development, every three months for the first year, and every six months thereafter; weekly notes summarizing progress, response to at least one specific treatment intervention in the IRP, participation in PC activities, and significant events will be completed by the end of the following week.
  • Collaborates with medical staff to ensure a seamless system of care.
  • Collaborates with staff and consumer in determining the most appropriate treatment program.
  • Acts as primary liaison to the State Hospital and Intensive Case Management and arranges for brief visitors
  • Acts as liaison with referral agencies, County Board of Social Services and the Law Project.
  • Supervises case managers who support engagement of new and high risk consumers.
  • Functions as case manager for new consumers until they are assigned a primary counselor.
  • Links consumers to appropriate entitlement sources, including General Assistance, Medicaid, food stamps, housing, medical and food banks.
  • Compiles and reports on referral, admission and retention data on a monthly basis.
  • Present all new case information to treatment staff at weekly staff meeting and as needed.
  • Maintain working relationships with all referral sources, including hospitals, agencies and physicians.
  • Maintain established productivity benchmarks.
  • Performs other duties as required.

 
Position Requirements

Qualifications:

  • Must be LCSW or LPC
  • Minimum of two years diagnostic and clinical experience in a psychiatric setting.
  • Ability to work well with serious mental illness and dually diagnosed consumers with serious mental illness, dual disorder diagnosis, and trauma histories.
  • Strong diagnositic, clinical, and engagement skills.
  • Strong organizational and documentation skills.
