Description

CLASS SUMMARY:

This class provides direct care, training and service plan development/coordination for adults with intellectual disabilities and who may have other disabilities including physical, cognitive, or mental disorders.

DISTINGUISHING ELEMENTS OF THE CLASS:

I. IMPACT: Immediate health and welfare of client is directly affected by work of this class. Activities of incumbent can promote successful community living for the client and assist in preventing unnecessary institutionalization.

II. LATITUDE: Day-to-day work is performed with regular guidance, consultation and frequent follow-up from the Residential Supervisor. Work is guided by comprehensive service plan, program policies, guidelines and instructions.

III. RESPONSIBILITY FOR THE WORK OF OTHERS: None.

IV. INTERPERSONAL CONTACTS: Continuous contact with clients and other staff in planning and providing client services. Regular contact with family members and members of other service agencies providing other services to clients, such as social workers, physicians, and case managers.

V. DIFFICULTY: Appropriately relating to a variety of client personality types requires use of judgment and discretion. Emergency situations resulting from client medical or behavior problems involve timely and responsible decision-making. Incumbent must deal with a variety of other human service officials complicating service plan delivery.

VI. COMMUNICATIONS: Work requires incumbent to prepare written documentation such as: reports, service plans, business communications, client progress notes, etc. Public relations activities are frequently involved.

VII. EQUIPMENT/FACILITIES RESPONSIBILITY: Responsible for maintaining and securing residential facility and associated furnishings and supplies.

VIII. WORKING CONDITIONS: Incumbent is assigned to a residential facility during scheduled shift. Work may also involve providing services in community settings. Some clients can be verbally and physically abusive.

IX. KNOWLEDGE, SKILLS AND ABILITIES (KSA'S):

Good knowledge of intellectual disabilities, autism, and other developmental disabilities.

Demonstrated skill in working with disabled adults and/or children.

Knowledge of and experience in the implementation of person-centered practices

Good oral and written communication skills.

Ability to establish and maintain effective relationships with clients and staff.

X. LICENSURE: Valid Virginia driver's license.

XI. PRINCIPLE DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Assists in preparing annual treatment plans and quarterly reports; provides input into overall residential programs.

Implements individual service plans; counsels clients; acts as role model for clients.

Communicates with the Residential Supervisor/team to make recommendations about person-centered plans/activities when an individual demonstrates a need/want for a change, and assists in ensuring the delivery of person-centered services

Interfaces with other human service officials engaged in providing services to client (social workers, case managers, physicians, etc.)

Participates in securing and maintaining orderliness of facility.

Maintains records and reports on clients including financial records.

Assists clients in learning daily living skills.

Takes inventory of facility supplies and equipment as needed.

Does related work as required.

Transports clients as necessary.

XII. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment; such as, but not limited to a personal computer, calculator, copier, and fax machine. Walking up and downstairs, pushing, and lifting a wheelchair, lifting, moving, and carrying boxes, office supplies, groceries, some climbing, bending, kneeling, crawling, reaching, and handling, sitting, standing, pushing, and pulling. May be required to move clients from one area to another.
