Description

This position is responsible for providing social work services to home health and hospice patients and their families.

Ensures the efficient delivery of social work services, hereinafter referred to as 'services' while at a minimum adhering to state and federal laws, state and federal rules, accrediting body standards, ethical standards, agency policies and procedures, the agency's mission and values, the agency's code of conduct, and business decorum.

Conducts social work visits

  • Identifies self as a representative of the agency to patients/clients and their families at the start of visits.
  • Provides services, including formulating a psycho-social assessment based on an evaluation of the environmental, psychological, familial, and health factors; impact of illness on patient/family, adaptive-coping mechanisms, specific stress areas, and financial difficulties including eligibility for public assistance and other insurance programs or private community resources.
  • Develops a social work treatment plan based on evaluation; goals assessed in consideration of realistic appraisal of patient/family's ability and/or wish to participate. Collaboration established with nursing/specialists or health care team in carrying through plan. Implements individual patient/family counseling as appropriate.
  • Locates, establishes, and maintains information and liaison with community resources. Makes appropriate referrals or assists health team members in same. Intervenes or serves as advocate with community resources as necessary on patient behalf to procure services.
  • Confers with nursing and other rehabilitation staff regarding patient/family needs including: psycho-social, financial, medical and environmental; discusses resources and/or options available to patient. Makes shared visits as necessary.
  • Ensures the accurate documentation of visits in a timely manner.
  • Develops goals and interventions that are to be included in the Plan of Care to address patients/clients specific spiritual needs.
  • Exemplifies and promotes positive, professional, and effective communication between and among disciplines. Ensures report is given to all disciplines involved in care prior to their initial visit.
  • Acts as the point of contact for patients/clients, their families for matters of the social work treatment plan.

Conducts death and dying visits.

  • Contacts the coroner, funeral home, and other individuals as necessary, when a patient/client dies.
  • Ensures the accurate completion of all documents in a timely manner.
  • Ensures the accurate documentation of discharge visits in a timely manner.

Promotes education for home health care staff members, patients/clients, patients/clients families and the community.

  • Participates in the development and implementation of educational programs to address identified community, patient/client or staff member need.

 
Position Requirements

Educational Requirements / Work Experience

  • Master's level graduate of an accredited school of social work REQUIRED.
  • License to practice social work in the State of Illinois REQUIRED.
  • At least one (1) year of experience as a social worker in a health care setting REQURED.
  • Previous experience in a home health setting PREFERRED.
  • Previous experience with the death and dying process PREFERRED.

  • Ability to deal effectively with stress.
