Share Email Opening Title Day Program Manager  Salary   Full-Time/Part-Time Full-Time   Shift   Location Boundless - West Carrollton 45449   Position Day Program Manager   Category Healthcare   Exempt/Non-Exempt Non-Exempt   Open Date 4/26/2018   Close Date 2/11/2019   Req Number HEA-18-00041   Description Classification Exempt Reports to Day Program Administrator Summary/Objective This position will provide program leadership and direction for the Boundless Day Habilitation Program in Montgomery County, Ohio – including Day Habilitation and Non-Medical Transportation Services – for adults with developmental disabilities. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Responsible for the management and supervision of the direct support professionals, quality assurance, and implementation of person centered planning for all individuals attending the Day Habilitation Program. Oversee and manage daily services for individuals. Ensure a healthy and safe environment for both individuals and staff members Plan and implement programming for the workshop, including a high level of community engagement for those who are interested. Assist clients with developmental disabilities in attaining the optimal level of independence Ensure legal compliance with DODD regulations, and client Individual Service Plans (ISPs) Document and report to the required entities any incidents adversely affecting the health and safety of a client Review incident reports and complete any follow up needed to ensure health and welfare. Directly supervise the Habilitation Professional positions in their oversight of of those served. Connect with potential and current community partners to enhance the community inclusion of individuals served. Ensure standards of CQL are maintained and exceeded. Ensure implementation of all sections of each individual's ISP As assigned, assist in the general referral, admission and transition of persons served into and within the organization via participation in staff meetings. Complete intakes, tours, assessments, and ISP’s for each individual to ensure person centered service delivery. Attend staff meetings, and other meetings, as assigned. Complete all required documentation related to funding, employment or general operations Assure that proper procedures are followed in regard to individuals and their inherent rights Assist in the timely submission of all records, files, financial records, etc Perform all other duties as assigned Competencies Leadership Humility Customer Service Collaboration Skills Results Driven Communication Proficiency Teamwork Orientation Thoroughness Flexibility Supervisory Responsibility This position directly supervises the Habilitation Staff positions. Required Education and Experience Positive attitude and enthusiasm for providing high-quality services to individuals with developmental disabilities. Bachelor’s degree in Human Services from an accredited College or University Three years experience in program management and supervision in the developmental disabilities field, preferably in day habilitation services. Valid Driver’s License and Insurable Driving Record. Demonstration of excellent oral and written/ documentation skills with strong teamwork skills. Ability to pass a background and drug screening meeting Ohio Adm. Code 5101:3-45-11 for Disqualifying Offenses.   Position Requirements Preferred Education and Experience Five to 10 years experience in program management and supervision in the developmental disabilities field, preferably in day habilitation services. Additional Eligibility Qualifications Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with consumers and other staff. Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility. Must possess the ability to make independent decisions when circumstances warrant such action. Work Environment This job operates in an office environment, but with frequent travel and community engagement. Physical Demands While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee is required to lift, bend, push, pull up to 50lbs. Position Type and Expected Hours of Work This is a full-time, exempt position, and hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional evening and weekend work may be required as job duties demand. Travel Travel is primarily local during the business day..
