Description

PURPOSE:
The Certified Peer Counselor supports the multidisciplinary team of the Crisis Triage Facility by providing and tracking discharge-planning services that are complex, multi system, and specific for the people we serve on the inpatient units. Using lived experience, Certified Peer Counselor focuses on providing hope and discussing practicality with the individuals supported. Having made use of various resources, the Certified Peer Counselor assists the discharging person with navigation of external resources.

RESPONSIBILITIES AND DUTIES:

  • Coordinate and facilitate timely implementation of discharge plans for clients. Develop and coordinate discharge plans that are: complex, multi system, mixed funding, and specific to clients.
  • Share lived experience when clinically appropriate to offer hope to clients currently in crisis, and to support in client building rapport with treatment providers.
  • Track the client's progress upon discharge at 7 days, 30 days, and 90 days by completing post-discharge follow up calls.
  • Maintain effective communication with physicians, nursing staff, individuals served, families and other related to discharge planning.
  • Coordinate with social services personnel to provide services as indicated by treatment team.
  • Document findings, discharge plans and action taken on medical record according to KMHS guidelines; prepare reports and maintain records as required.
  • Maintains the clinical information of individuals served by the unit in a confidential manner.
  • Attends orientations, staff meetings and keeps all yearly mandatory trainings up-to-date as required by the unit.
  • Supports the Mental Health Professionals and Recovery Case Manager is in tasks such as arranging for shelter beds, and follow up appointments.
  • Participate in shift pass downs as needed to ensure that client's needs are met 24/7 while at the Crisis Triage Facility.
  • Track the total number of all discharges from the unit to ensure that client's needs were met and that discharged clients are able to continue to access supportive services.
 
Position Requirements

MINIMUM QUALIFICATIONS: Current WA State Peer Counseling certificate or complete certification within six months of employment. Preference for lived experience in an inpatient or crisis setting. Ability to gain licensure as an Agency Affiliated Counselor (or other applicable counselor profession) in Washington State, valid Washington State Driver License and Food Handler card. High School Diploma or GED required. A bachelor’s degree in a counseling-related field is preferred. Ability to provide client transportation in an agency vehicle.

PHYSICAL CONSIDERATIONS: While performing the duties of the job, the employee is required to stand, sit, use hands and fingers, reach with hands and arms, ascend and descend stairs, talk or listen and lift up to 25 pounds. Peripheral vision is also required for this position.
