Being a Blended Case Manager has its Advantages -As a Blended Case Manager at Devereux’s Children’s Behavioral Health Services you will work with other dedicated professionals who share your passion for helping individuals in need. This position will give you an opportunity to truly make a difference in someone’s life by advocating and providing problem resolution for needed services to education, housing, mental health, employment, healthcare, legal aid and social skills. .
Devereux Blended Case Management aims to help children 2-21 years of age and their families to navigate the world of Social Services. We work collaboratively with family members, funders and service providers to advocate on behalf of the child, ensuring care is coordinated and is effective and responsive to the needs of each child we serve.
The home office is located in Phoenixville, PA however this position also works in the community and visiting the homes of assigned clients. Vehicles are provided for work travel.
-Develop and revise service plans every 6 months based on client’s strengths and needs.
-Coordinate care for mental health, medical, educational, placement and related areas.
-Maintain direct contact via telephone and face to face with caseload and parent/guardian.
-Serve as a liaison capacity communicating with the client, the family/guardian, agency representatives and other stakeholders.
-Ensure timely and accurate completion of related documentation.
-Assist in hospitalization process as required.
