Boundless - Step By Step campus is currently looking for a highly motivated Licensed Social Worker to work as a Center Based Clinician for our Worthington location.
Summary/Objective The social worker reports to the clinical manager and is responsible for ensuring that the medically related emotional and social needs of the clients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state and local standards, guidelines, and regulations.

Develops a comprehensive social history and psychosocial assessment that includes the client's problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan.

Helps clients and their families identify needs and develop service plan to meet those needs.

Orients clients and families to the program, its services, its service limitations and clients' rights.

Involves clients and families in care planning, encouraging their attendance at care conferences.

Attends scheduled care conferences and identifies and interprets psychosocial needs of clients for inclusion in the care plan.

Completes mandatory documentation within timelines established by applicable regulations and policies.

Mediates issues that arise among clients, families and staff.

Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, etc.)

Provides clients and families with crisis management services as needed.

Develops a comprehensive discharge plan in collaboration with clients, clients' families, multidisciplinary staff, and referral agencies.

Trains and consults with staff regarding psychosocial needs that encompass mental health, development, and diversity of individual clients to increase staff knowledge and skills.

Educates staff about any relevant state legislation or regulations relating to behavioral health services.

Advocates on a case, policy and program level.

Participates in policy decision making in a way that encompasses an understanding of the issues and implications relevant to the developmental and behavioral health needs of clients served.

Meet required productivity requirements of 22.5 billable hours per week.

Provide clinical review and audit of clients' charts and documentation to ensure compliance and support quality improvement.
Competencies
Supervisory Responsibility
This position has no supervisory responsibilities, but works regularly with the Behavior Technicians providing oversight of their work. This position will provide training and consultation to Behavior Technicians, and assist in developing and monitoring training plans for Behavior Technicians.
Work Environment
This job operates in a professional office environment, but regularly traverses the campus in order to provide support in the program buildings. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.
Position Type and Expected Hours of Work
This is a full-time position, and hours of work and days are Monday through Friday, 8:30 a.m. to 5 p.m. Occasional evening and weekend work may be required as job duties demand.
Travel
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
