Description

JOB PURPOSE: Provide case management services for lodge members living with a serious mental illness specifically in the areas of psychiatric, medical, and entitlement needs. Assist members in obtaining and learning to maintain an acceptable level of care for their overall well-being from multiple service providers.

SCHEDULE: 24 Hours a week and there is some flexibility to the schedule, but usually either:

  • Tues-Fri: 8:30-3:30 with a lunch
  • Mon-Fri: 9-2:30 with a lunch

ESSENTIAL JOB RESPONSIBILITIES:

50%- Responsible for all the issues related to psychiatric, medical and benefits. Responsibilities are carried out by advocating, teaching, training, supporting, coaching, and acting as a check and balance including, but not limited to, the following areas:

  • Psychiatric Services
  • Medical Services
  • Medication Management
  • Benefit Acquisition/Maintenance: Insurance, SSI/SSDI, VA, etc
  • Dental Services
  • Optometry Services

10%- Assist in interviewing and evaluating potential clients to determine eligibility into the program. Monitor/evaluate/document progress and growth of lodge members' progression through the psychiatric, medical, and benefit aspects/needs of the lodge member including, but not limited to:

  • Psychiatric: Appointments, Symptom Management, Coping Skills, etc
  • Medical: Appointments, Annual Physicals, Medical Conditions
  • Medication Management: List of all medications, Side Effects, Medication Interactions, etc
  • Dental Needs: Standardized cleaning schedule, surgical issues, etc
  • Optometry Needs: Appointments, Needs for Glasses, etc.

5%- Document monthly and emergency case notes; submit comprehensive resident files, update charts as needed. Prepare necessary incident reports.

10%- Identify and provide emergency crisis services as necessary; make immediate clinical assessments and respond according to accepted crisis intervention methods and techniques; coordinate other services as appropriate.

20%- Empower lodge members to learn how to maneuver through the multiple service providers they encounter to increase their individual level of independence. Assist them in learning how to properly utilize the services that are available to them as well as communicate their needs to the service providers. Assist the lodge members to make decisions by following the Fairweather Model using the group problem solving process.

5%- Monitor quality and effectiveness of resident services. Work with outside agencies such as APS, Hospitals, Partial Hospitalization Programs, DARS, Probation, etc. to ensure residents' success.. Provide on-going program evaluation and recommendations to the Lodge Coordinator for continuous growth and quality.

Position Requirements

ESSENTIAL QUALIFICATIONS:

EDUCATION: Associate’s Degree in Human Services field. Bachelor’s Degree in Social Work, Sociology, Psychology or related field preferred.

EXPERIENCE: No case management experience required with related Bachelor’s Degree, 1+ year experience w/o Bachelor’s Degree, 3+ years preferred. 2+ years in a customer service focused environment. Experience with mental health and/or a homeless population a plus.

LICENSES: Drivers License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.

EXCEPTIONS TO THESE CRITERIA MUST BE APPROVED BY THE DEPARTMENT HEAD.

ESSENTIAL KNOWLEDGE, SKILLS, and ABILITIES:

COMMUNICATION: Ability to communicate clearly and effectively via oral or written means. Ability to present a friendly and positive demeanor to residents, staff, supportive services personnel, funders, and the general public. Ability to make oral presentations.

COMPOSURE: Ability to remain calm and maintain self-control in the midst of difficult circumstances and emergencies. Ability to respond in a professional manner in all situations.

COMPUTER: Working knowledge of Word, Excel, Outlook. 45 WPM, 90% accuracy.

DECISION MAKING: Ability to think analytically and evaluate the impact of case management decisions. Ability to set work priorities and to evaluate and create solutions to work related problems.

FINANCIAL: Strong math skills. Ability to assist residents in developing a budget.

NEGOTIATION: Ability to negotiate clients down from stressful situations, potential threatening behaviors. Ability to persuade clients to make appropriate life decisions.

TECHNICAL: 2+ years case management experience a plus. Knowledge of case management services; City and County court systems, community resources, and disability benefits.

MACHINES, TOOLS, EQUIPMENT, WORK AIDS: Multi-line telephone, computer, ten key calculator, printers, copiers, faxes.

PHYSICAL REQUIREMENTS:

Constant – Ability to communicate with staff, residents and support services to determine needs and actions to be taken.

Frequent – Ability to observe clients to evaluate mental, emotional and physical condition.

Frequent – Ability to input data into computer, make calculations, complete paperwork, produce reports.

Occasional – Ability to bend, kneel, reach or stoop to perform lodge inspections and demonstrate life skills.

Occasional – Ability to lift, stack, load and move up to 25 pounds from varied heights.

Occasional – Ability to drive safely and recognize hazards.

WORKING CONDITIONS:

Indoors: 85 % - Office and residential environment; generally normal noise level

Outdoors: 15 % - All weather conditions and variable temperatures.
