Program Facilitator (ILS)

Independent Living Skills (ILS) Program Facilitators provide support and instruction to participants with intellectual & physical disabilities in community, home and classroom settings. ILS Program Facilitators also provide administrative, operational and personnel support to the ILS Program Manager (PM).


Administrative Support

  • Develop and implement current, functional and age-appropriate curriculum for participant activities.
  • Develop, implement, modify and review data tracking sheets for each participant.
  • Maintain information in the Program Data Management System.
  • Create, maintain and update field packs, new participant packets, participant files/records and case files.
  • Complete Special Incident Reports (SIR) and submit to the appropriate agencies/persons in the PM's absence.
  • Track, calculate and compile monthly, semi-annual and annual data.
  • File, purge and archive program information and forms, per program guidelines.
  • Prepare for and follow-up on case file review corrections.
  • Support and assist participants in completion of recertification process and other housing related matters.
  • Provide transportation to individuals, as needed.
  • Assist in teaching/co-teaching tenant meetings, classes and events, as needed.
  • Complete and submit billing support information.
  • Take notes at Program Meetings and distribute/file, as needed.
  • Assist with the completion of programmatic reports, documentation, forms and paperwork.

Operational & Programmatic Support

  • Provide independent living training, support, supervision and instruction to participants in a variety of settings.
  • Ensure that the participants' training schedules are followed and documented.
  • Register individuals at various community colleges.
  • Perform and document spot checks.
  • Complete entire ISP process including writing, mailing, attending and follow-up.
  • Oversee completion of PFPs for each participant.
  • Establish data tracking and other supports for participants.
  • Collaborate with Interdisciplinary Team (IDT) individuals.
  • Function as acting Program Manager, when necessary.
  • Serve as a Safety Representative on the Safety Committee.
  • Diffuse crisis situations and provide crisis intervention, as necessary.
  • Conduct safety drills with staff and participants.
  • Assist with supervision and training of staff; report potential staff issues to Program Manager (PM).
  • Follow-up with staff deadlines for paperwork responsibilities.
  • Effectively relay issues and concerns to the Program Manager.
  • Attend all required meetings and in-services.
  • Complete all required training.
  • Support fundraising activities of the organization.
  • Comply with all Employment & Community Options policies and procedures.
  • Conserve company resources.
  • Other duties, as assigned.
Position Requirements


  • High School Diploma, equivalent or GED.
  • Bachelor’s Degree in Human Services or related area (preferred).


  • Two years hands-on experience with individuals with a wide range of intellectual & physical disabilities, including individuals with the most severe challenges.
  • Experience in community-based instruction in critical life needs areas, with emphasis on functional and age-appropriate skills (preferred).
  • Experience in assisting individuals in the use of adaptive devices and communication aids (preferred).

Required Skills

  • Ability to communicate effectively both orally and in writing.
  • Ability to conduct training.
  • Professional English writing ability, including usage, grammar, spelling, punctuation and vocabulary.
  • Proficient use of basic office equipment, Microsoft Word and Excel.

Other Requirements

  • Immediate access to a vehicle with working seat belts, valid driver’s license, current auto insurance and acceptable DMV record.
  • Ability to travel within and throughout the assigned program area in which services are provided.
  • Successful completion of drug screen and background check.
  • CPR/First Aid Certification (preferred).


This position requires the employee to regularly stand, walk, sit, talk and hear. They must be capable of writing, using a computer keyboard, telephone and related office equipment. Ability to lift 25lbs required. 

The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

The employee will be working in community environments, employment sites and office environments. The employee will experience normal office conditions with a well-lighted work area and minor noise from standard office equipment. Exposure to blood borne pathogens could occur when administering first aid to participants. Hepatitis B vaccines are offered at no cost to each employee.

WAGE: Wage range is between $18.50 - $21.50 per hour + benefits. Actual wage is based upon level of education and experience.
