SHIFT: 20hrs per week (Monday, Wednesday, Thursday)

The Program for Assertive Community Treatment is a multidisciplinary team which utilizes the strengths of individual team members in assigning tasks, with the philosophy that staff members, regardless of qualifications are somewhat interchangeable around team task fulfillment.  The Psychiatric Nurse is responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and support services; and sharing shift management responsibility with other staff.  It is an expectation that the PACT Team Psychiatric Nurse will provide most services in community-based settings.


  • May be asked to provide treatment coordination services for an assigned group of clients, including coordinating and monitoring the activities of the individual treatment team (ITT).  
  • May be asked to assume primary responsibility for collaboratively developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the client and the ITT. Use recovery-based concepts and language in both written and verbal communications. 
  • Provide individual supported therapy and symptom management. Utilize Cognitive Behavioral approach, Motivational Interviewing, stages of change theory, Trauma Informed Care approaches, and stage-wise treatment strategies to intervene and motivate clients for change.  
  • Conduct comprehensive assessment of psychiatric history (e.g. onset, course, and effect of illness, past treatment and responses, and risk behaviors), mental status and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g. self-care, living situation, nutrition, money management); and family structure and relationships.
  • Educate and support clients’ families and advocate for clients’ rights and preferences.  Consult with families and community agencies such as DSHS, court systems, housing authorities, etc., to maintain coordination in the treatment process.
  • Document client progress to maintain a permanent record of client activity according to established policies and procedures.
  • Participate in daily staff organizational meetings and treatment planning review meetings.
  • Under the direction of the team medical provider, and in collaboration with other registered nurses on the team, develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures including administering, evaluating, and recording prescribed psychotropic medications; evaluate and chart psychotropic medication effectiveness and side effects; and arrange for required lab work.
  • Coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians and dentists, and coordinating psychiatric treatment with medical treatment.  
  • Participate in providing information to clients on substance use issues such as the effect on mental and physical health and daily functioning.  Participate in planning to reduce use or abstain from the use of substances, and help teach replacement behaviors.  Establish linkages with outside inpatient, outpatient, and support group entities (e.g. AA, NA) to allow collaboration in treatment.
  • Provide or collaborate with teammates to ensure the provision of vocational-supportive counseling designed to help clients achieve vocational or career goals.  Provide ongoing support for job retention, while managing symptoms.
  • Provide and coordinate outreach and crisis intervention services to clients in the community, and routinely identify and evaluate physical risk factors for self and others while in potentially unsafe neighborhoods, homes, while around people with unknown potential for aggression, and while in proximity of potentially unsafe and/or unclean structures or equipment. Consult with DMHP’s and law enforcement officials when appropriate.
  • Assess for and provide psychiatric rehabilitation services. Assist clients in building skills for independent living in the community, including, but not limited to, shopping, cooking, cleaning, personal hygiene, budgeting, socialization, and accessing transportation.
  • Assist clients in finding and maintaining a safe, affordable place to live, to include supporting clients in household activities such as house cleaning and laundry.
  • Ensure that clients have adequate financial support (e.g., help to gain employment or to secure entitlement benefits).  Assist clients with budgeting and bill-paying skills.
  • Plan, structure, and prompt social and leisure time activities, to include evenings, weekends, and holidays.
  • Provide individual services for current and newly assigned clients while in inpatient programs, to ensure continuity of care.
  • Monitor assigned clients for LRA compliance.  Facilitate LRA extensions with the CRT and all court-involved personnel to insure LRA's are continued uninterrupted, as appropriate.  Assist CRT in determining when revocation is appropriate.
  • Maintain proficiency in current trends in mental health, including health care reform and information technology.  Maintain knowledge of PACT standards and Evidence Based Practices. Maintain a Trauma Informed Care work environment.

Position Requirements

MINIMUM QUALIFICATIONS:  Current license as a Registered Nurse in the State of Washington.  Two years RN experience preferred.  Previous experience working with the chronically mentally ill preferred.  Must have valid Washington State Driver’s License and reliable personal transportation with business-rated insurance. Possess and maintain valid driver's license with an acceptable motor vehicle report.

*All employees that drive personal vehicles for daily job functions must provide proof of auto liability insurance of a 100/300 policy upon the first day of employment, maintain required coverage amounts and provide ongoing documentation of continuous coverage.

PHYSICAL CONSIDERATIONS:  While performing the duties of the job, the employee is required to sit, stand, walk, use hands and fingers, reach with hands and arms, talk or listen and lift up to 10 pounds.  This position is also required to have close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.  The ability to physically assist clients who are physically disabled may also be required.
