We are seeking mission focused professionals to serve as a full time Case Coordinator responsible for the case management functions of a caseload of SAFY foster youth and respective foster homes in our Cleveland, OH division!

Essential Responsibilities:

  • Coordinate home visits for the youth with all appropriate parties.
  • Attend hearings and semi-annual review for all youth assigned to caseload. Assist with enrolling youth in school and attends Individualized Education Plan (IEP) meetings. Assure that health care needs of youth are being realized including medical, dental, vision and hearing as well as special needs.
  • Complete assessments as required by state rules, agency policies, and/or contract requirements.
  • Prepare Individualized Treatment Plans (due every 90 days per client), DAF's, and Mental Health Individualized Services Plans.
  • Timely document outcome of home visits, support sessions, interaction with parties involved in the youth's care and incidents as required by policy, regulations and contract requirements. It is approximated that one will have 10 hours of documentation a week to complete which is due within 72 hours of the service along with a productivity expectation of 30 hours per month.
  • On-call is on a weekly rotation and is approximately every 6-8 weeks.
 
Position Requirements

Education Requirements:

  • Bachelor's or Master's degree in the field of social work, counseling, psychology, sociology, child & family studies is required.

License Requirements:

  • LSW or LPC in the State of Ohio is required or must be LSW/LPC eligible.

Experience Requirements:

  • Two years of relevant experience in child welfare, mental health, human services, or social work is preferred.
