Description

Youth employment programs at Holland Bloorview support teens and young adults with disabilities to participate in employment experiences and build skills, confidence and connections. The Life Skills Coach will work as part of a team to facilitate several employment groups but will primarily work with youth who are participating in:

  1. Youth@Work: A summer program for youth ages 15-20 (with at least one more year in high school) who want to build skills for working or volunteering. Clients participate in supported work placements, group workshops and individual work preparation activities. Clients learn about their interests and strengths, and practice essential life skills including social communication skills, problem-solving, self-advocacy, and team work.
  2. Leading the Way: Supported co-op experiences at Holland Bloorview for high school and post-secondary students (September through June).

The Life Skills Coach collaborates with clients/families and team members to develop a coaching intervention plan that supports the client’s participation in work experience placements. This may include coaching for: performing job tasks, engaging in social and communication opportunities in the work place, building life skills, and/or addressing barriers to workplace participation. Team members may include other Holland Bloorview clinicians, teachers, employers, co-workers, etc.

1 year contract (option to renew)

Key Responsibilities

Clinical Practice

  • Collaborates with family and the interdisciplinary team to assess the client’s life skill needs, develops and evaluates a plan of care which is goal directed, evidence-based and outcome focused
  • Respects the client/family’s rights to make decisions based on their own values, beliefs and experiences
  • Bases practice within framework that includes: teaching problem solving skills, experiential learning approach, calculated risk taking and modelling related to self, family, community, job and leisure
  • Develops resources that can be used in the delivery of services
  • Evaluates program effectiveness and makes appropriate recommendations for change
  • Works flexible hours to meet client/family needs. Must be available to work evenings and weekends
  • Complies with documentation guidelines outlined by hospital
  • Documents accurate workload statistics
  • Attends and actively participates in staff meetings
  • Communicates in a timely and effective manner
  • Participates in quality improvement initiatives, supports the maintenance of a safe and healthy work environment and advances a culture of client/patient safety through work and daily practices

Professional Development/Leadership

  • Ensures the maintenance of the Standards of Care for Life Skills Coaches
  • Identifies learning needs and establishes goals
  • Identifies practice issues and participates in their resolution
  • Remains current with professional literature

Research and Best Practice(s)

  • Participates in research activities as applicable
  • Utilizes research information to inform evidence based practice
  • Other duties as assigned

Qualifications

  • Bachelor’s Degree in a health or social services field
  • Candidates with specialized Life Skills training, a background in Therapeutic Recreation, or experience working as a job coach will be preferred
  • Experience working in the field of education, health or social services, with adolescents who have disabilities (individually and in groups)
  • Lived experience of disability preferred
  • Written and verbal English language communication skills required to effectively communicate with clients, families and members of the interdisciplinary team
  • Proven ability to measure service effectiveness and efficiency
  • Demonstrates ability to organize time and meet deadlines
  • Demonstrates initiative and self direction
  • Demonstrates ability to effectively and efficiently manage fluctuating clinical responsibilities
  • Proven ability to be an effective member of an inter-professional team
  • Must be available to work evenings and weekends
  • Access to a vehicle preferred; willing to regularly travel to community locations for client service delivery
