The Occupational Therapist will work as part of a team to facilitate several employment groups but will primarily work with youth who are participating in: Ready to Work: A transition pathway to support youth ages 15-26 to search for - and start - a paid summer job. Job Finding Group: A group and 1:1 coaching service to support clients + a family member to make an action plan for their next employment participation step (volunteering, paid employment, self-employment). Leading the Way: Supported co-op experiences at Holland Bloorview for high school and post-secondary students (September through June). The Occupational Therapist collaborates with clients/families and team members to develop a intervention plan that addresses client and/or employer goals and needs. This may include aspects of self-care, community mobility and independence, life skills, workplace accessibility, learning and performing job tasks, workplace social participation and communication opportunities, etc. ace, building life skills, and/or addressing barriers to workplace participation. Team members may include other Holland Bloorview clinicians, teachers, employers, co-workers, etc. Contract 1 year (potential to renew). Flexibility to work some evenings/weekend shifts during the school year is required. Summer hours are typically week days and applicants must be available all July & August. Key Responsibilities: Partner with clients and families to formulate and implement individualized client-centered goals with focused, targeted and measurable outcomes. Provide client care in collaboration with the client/family to plan, implement and evaluate care, documenting the habilitative/rehabilitative process according to hospital policies. Interface and problem solve with members of the interprofessional care team and community providers as appropriate to ensure an integrated approach to care. Incorporate evaluative activities into professional practice to ensure the delivery of efficient and effective health care. Utilize research to establish and support evidence-informed practice. Utilize the teaching/learning process to provide education on various conditions, both formal and informal, to clients/families, colleagues, students, and other professionals within the organization and the community. Provide direct service, consultation, mediation and education to clients, families, and community providers with focus on maximizing the client's function in the areas of self-care, productivity (school and play skills) and leisure (community or sports participation). Address challenges identified in client occupational performance. Provide assessment and intervention directed at the person, occupation and environmental factors influencing occupational performance. Practice according to organizational policies and standards of practice as set out by the College of Occupational Therapists of Ontario. Participate in quality improvement initiatives, support the maintenance of a safe and healthy work environment and advance a culture of client/patient safety through work and daily practices. Other duties as assigned Qualifications: Masters degree in Occupational Therapy (or Bachelor degree in Occupational Therapy plus other Masters) Current registration with the College of Occupational Therapists of Ontario Membership in the Canadian Association of Occupational Therapists and/or the Ontario Society of Occupational Therapists is an asset.
