Community Care Liaison - Community Care Hospice & Hospice of Fayette County

The Community Liaison is responsible all sales and marketing activities, from lead generation through close in an assigned territory, developing and implementing agreed upon marketing plans which will meet the goal of expanding the customer base in the market areas. The Community Care Liaison works within the sales team, and with the clinical teams, for the achievement of customer satisfaction, and the continual growth of census. This is shown by meeting team goals for referrals on a monthly and annual basis. This will be achieved through long term development of account relationships, in line with Ohio’s Hospice vision and values.

Key Job Responsibilities, Outcomes and Standards:

Generate referrals and admissions through sales and marketing strategies, within assigned territory. Territories being; hospitals, physician offices, home health agencies and community health organizations.

Develop strategies for assigned area that focuses on growth and sustainable growth. Also being flexible, to recognize changing referral patterns in assigned area which will result in modifying strategies, to meet on going challenges.

Minimum of 40 account visits per week. Community Care Liaisons will be monitored through the net-track system weekly by director to ensure that standard numbers of visits are being made to provide customer satisfaction and referral growth. Documentation is completed in CRM at the end of each business day. Director will closely monitor three times per week for completed documentation to ensure up-to-date information is readily available to facilitate effective communication and timely follow-up with accounts.

Responsible for meeting monthly and annual referral/admission goals in a specific territory. Monthly team referrals/admissions goals will be accomplished 90% of the time. Monitored monthly by Director through Net-track System.

Identifies, develops and maintains key relationships in the medical community, by participating in various functions sponsored in the medical community. Community Care Liaison will involve themselves in one to two meaningful community activities within service area (serving on organizational boards, volunteer activity, and participation in an organization’s annual event.) Community Care Liaisons will inform the Director which events they will be attending.

Promotes Ohio’s Hospice through educational opportunities and community events. Community Care Liaison will provide educational opportunities in assigned area. Also will attend community events sponsored by Ohio’s Hospice, as assigned by director.

Will assist with informational visits as requested.

 
Position Requirements

The appropriate candidate will be results oriented and have a passion for getting the hospice benefit to all eligible patients.

  1. Valid Ohio Drivers License
  2. A broad understanding of the sales process in the Healthcare Industry
  3. Bachelor’s Degree, in business, marketing or a combination of education and experience.
  4. 2 years sales experience preferred
  5. Computer competency: Basic knowledge along with MS office and CRM
  6. Understanding and experience with the Hospice Concept and Philosophy a plus.
  7. State minimum auto insurance
  8. ** A Safe driving record is mandatory: no more than 3 moving violations or more than one chargeable accident in the past 36 months and no major convictions (drunk, drug or reckless driving) within the past 7 years.
