GENERAL DESCRIPTION OF POSITION
Prepares and provides assistance to Pathfinder Registered Nurses, staff, individuals served, parents, and/or legally responsible person of the consumers we serve. Administers medication and renders first aid and subsequent treatment to injured or ill clients as needed and qualified to perform.

ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Verify change out list is correct and up to date, notifies pharmacy of changes.
2. Verifies doctor's orders are filled correctly by pharmaceutical company before delivery to site.
3. Assists in the design and integration of individual objectives as part of the overall service delivery process.
4. Provides information and assistance to persons responsible for developing the consumers' service plans.
5. Keep up with immunizations, blood level appointments, and preventative test for individuals served.
6. Coordinates required quarterly documentation to DDS for Medicaid Waiver (Nurse) Consultation as directed.
7. Coordinates and tracks proper documentation for services that are billable under ACS Waiver Consultation.
8. In accordance with the Nurse Practice Act, coordinates tracks appropriate paperwork regarding The Consumer Direct Care Bill as directed by the Director of Residential Services.
9. Will track client medication monitoring training with new staff to ensure client competence in self administration of medication and training adherence.
10. Tracks Personal Care Program by providing training to Personal Care Aides for certification, provides additional continued education hours required for re-certification, completes Personal Care Assessment and Service Plan, and completes Personal Care record reviews and program observations.
11. Provides training to individuals served on health, hygiene, and sanitary living conditions.
12. Will assist in selection of additional nursing services as needed or desired.
13. Provides and maintains documentation of current professional license to be on file with Pathfinder, Inc.
14. Will work in a cooperative team spirit with all Pathfinder, Inc departments and employees, parents, legal representatives, and consultant staff.
15. Attends meetings and training sessions as directed.
16. Administers prescribed medications, injections and treatments as plan requires.
17. Gives tube feedings as necessary if plan required.
18. Visits residents three to four times a day to administer medications, monitor medication given, and chart medications if required by individuals plan.
19. Notifies family of infectious diseases.
20. Perform any other related duties as required or assigned.
