POSITION: CLINICIAN II (EISS) JOB SUMMARY: This position is responsible for the coordination and delivery of clinical behavioral health services designed to meet the clinical wellness and recovery needs of the consumer. Depending on consumer needs/goals and objectives, these services may include resource development, assessments and therapy (individual, group and family) . ESSENTIAL FUNCTIONS Intake: Conduct initial and ongoing assessments involving the collection, analysis, and recording of significant bio-psychosocial information. Makes diagnoses Clinical Services: Provide individual, family and group counseling to consumers as needed. Provides information sessions for individuals and groups to reduce resistance to mental health treatment and enhance engagement Collects specimens for drug use analysis Maintains regular therapeutic contact with clients Treatment plans for clients on medication prescribed by program staff will be done in consultation with the prescriber. Documentation: Enters reports and assessments into information technology system as indicated by the Program Director Maintains accurate and up-to-date case records in accordance with agency standards, accreditation and funding source requirements Case Management: Coordinates treatment with relevant staff both inside and outside the agency Advocates for consumers and makes indicated referrals Productivity: Clinicians are expected to spend 65% of their time in face to face consumer contact and to provide routine productivity reports to their supervisor. All schedules, statistical reports/logs, and clinical reports are to be submitted within proscribed timelines. Documentation should be completed in Anasazi within 24 hours of service delivery. OTHER DUTIES Participate in the Quality Improvement process and specific quality initiatives for the program. Utilize supervision and training as opportunities for enhancing professional competence. Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, and professional standards. Must work 1 – 2 evenings a week or 1 evening and a Saturday as required by caseload and office hours of operation. Other related duties as assigned. EISS Specific responsibilities: Participates in Mobile Outreach responses for the initial evaluation and referral of consumers to the EISS program Provides crisis intervention |
