Description

Ensures the efficient delivery of occupational therapy services, hereinafter referred to as 'services' while at a minimum adhering to state and federal laws, state and federal rules, accrediting body standards, accepted occupational therapy practices, ethical standards, agency policies and procedures, the agency's mission and values, the agency's code of conduct, and business decorum.

  • Implements patient/client occupational therapy regiment pursuant to the established plan of care and conducts visits.
    • Accurately completes weekly schedule and conducts visits ensuring compliance with established visit frequencies.
    • Reviews the plan of care to coordinate care with the Occupational Therapist and other healthcare providers.
    • Identifies self as a representative of the agency to patients/clients and their families at the start of visits, as necessary.
    • Ensures the start and completion of visits are independently documented using telephony or other prescribed method.
    • Ensures the accurate and efficient procurement of medical supplies in a timely manner.
    • Follows the prescribed occupational therapy treatment program for patients/clients to restore function and prevent disability following disease, injury or loss of body part.
      • Completes the plans program involving activities, such as manual arts and crafts; practice in functional, prevocational, vocational, and homemaking skills, and activities of daily living; and participation in sensorimotor, educational, recreational, and social activities designed to help patients/clients or handicapped persons develop or regain physical or mental functioning or adjust to handicaps.
      • Completes the plan of care by overseeing constructive activities suited to individual's physical capacity, intelligence level, and interest to upgrade individual to maximum independence, prepare individual for return to employment, assist in restoration of functions, and aid in adjustment to disability.
      • Teaches patients/clients skills and techniques required for participation in activities and evaluates their progress.
    • Works to maximize patient/client outcomes.
      • Independently identifies educational needs of patients/clients and their families related to patients'/clients' specific care needs and ensures patients/clients and their families are timely and adequately educated.
      • Observes, documents, and reports to the Occupational Therapist on the patient's/client's treatment, response and progress in a timely manner.
      • Identifies, recommends, documents and works with supervisor to implement changes to the plan of care as approved by patients'/clients' physicians in a timely manner.
      • Ensures the utilization of available technologies, such as telehealth.
      • Ensures infection control measures are strictly adhered to.
      • Provides ongoing education to patients/clients and their families as necessary and appropriate.
      • Continually works to improve job-related knowledge, skills, and performance.
      • Coordinates with other staff, including, when appropriate, home health aides and family members, in certain phases of occupational therapy with which they may work with the patients/clients.
    • Ensures the accurate completion of all visit documents in a timely manner.
    • Ensures the accurate documentation of visits in a timely manner.

 
Position Requirements

PROFESSIONAL LICENSE/CREDENTIAL REQUIREMENTS/PREFERENCES

  • Graduate of an accredited program in occuaptional therapy assistant REQUIRED.
  • License to practice as am occupational therapy assistant in the State of Illinois REQUIRED.
  • Certified to perform Cardio Pulmonary Resuscitation (CPR) PREFFERED.  Certified to perform CPR within six (6) months of hire REQUIRED.

WORK EXPERIENCE REQUIREMENTS/PREFERENCES

  • At least one (1) year of appropriate experience practicing occupational therapy PREFERRED.
  • Previous experience in a home health setting PREFERRED.
